NANOPARTICLES: PHARMACOLOGICAL EXPECTANCIES AND TOXICOLOGICAL PROBLEMS

The aim of the study was to get familiarized with one of the most intensively developing fields of medicine at present – nanomedicine, which is based on the usage of nanoparticles for diagnosing and treating different diseases. The main types of nanoparticles used in medicine are characterized and their significance in treating neoplastic and other diseases is underlined. At the same time attention is brought to possible toxicological problems, which may occur if nanoparticles will be uncontrollably used in human life.
 

STRUCTURAL AND GEOMETRICAL PARAMETERS
OF THE RIGHT HEART VENTRICLE

The structural and geometrical parameters of the right heart ventricle have been studied in healthy people with different constitution types, and the most informative index was established. The results show that the size of the right heart ventricle has most significance in hypersthenic constitution type and least significance in asthenic constitution type. People with normosthenic constitution type hold the middle position. The criteria for normal right ventricle sizes for different constitution types are proposed.
 

PROGNOSIS OF FATAL CARDIOVASCULAR COMPLICATIONS IN PATIENTS WITH ARTERIAL HYPERTENSION OF THE I-II STAGES

A cohort controlled 15-year prospective clinical and functional biochemical study has been conducted which included 292 people 40-59 years of age (232 in the main group and 60 in the control group). During these years, three people died from cardiovascular reasons in the control group, and 16 people died in the main group. Using multiple-factor analysis three mortality risk factors from cardiovascular disease have been established: cholesterol level higher than 5,8 mmol/l, male sex and thickness of the posterior wall more than 1,1 cm. A formula for calculation of the possibility of developing fatal cardiovascular complications in 15 years time in patients with arterial hypertension was proposed.
 

THE PECULIARITIES OF URATE METABOLISM IN ACUTE BRAIN ISCHEMIA

348 patients were investigated in the acute period of cerebral ischemic stroke. It was found that there is a dependency between the parameters of uric acid diathesis and cerebral ischemia. The ischemic brain lesion was accompanied by significant elevation of uric acid concentration in cerebrospinal fluid and moderate elevation in venous blood, which reflected the severity of neurological deficiency, disorders of cerebral metabolism and was associated with a fatal outcome. The mortality was more than twice higher in patients with initially high concentration of uric acid in liquor than in patients with normal concentration of uric acid, while the combination of high uric acid concentration both in blood and in liquor thrice increased the mortality rate.
 

MARKERS OF THROMBIN AND FIBRINOGEN INTERACTION IN BLOOD PLASMA AS INDICATORS OF THROMBIN TOLERANCE

The probability of thrombin fibrinogen interaction rate and tolerance to thrombin was studied in rats. The significant negative correlation between the intensity of thrombin fibrinogen interaction and the ability of the organism to tolerate hyperthrombinemia was established. The possibility of using thrombin fibrinogen interaction markers detection in evaluating hyperthrombinemia tolerance was proposed.
 

THE CORRECTION OF MICROCIRCULATION DISORDERS USING LOW INTENSITY LASER THERAPY IN PATIENTS WITH PNEUMONIA

Microcirculation and several indices of hemocoagulation after low intensity laser complex treatment of patients with pneumonia were studied. 32 patients of the control group received traditional drug therapy while 66 patients of the main group besides the same drug treatment received a course of blood laser exposure. Intravenous blood laser exposure came out to be an effective method for correcting hemocoagulation and microcirculation disorders in patients with pneumonia.
 

COMPUTER AND MAGNETIC RESONANCE ANGIOGRAPHY IN DIAGNOSING OF THE PULMONARY EMBOLISM

Efficacy of spiral computer tomographic and magnetic resonance angiography to diagnose the pulmonary artery thromboembolia was evaluated in patients with surgical diseases. It was found that magnetic resonance angiography is a good alternative in case of absolute contraindications for spiral computer tomographic angiography.
 

ANASTOMOSIS FORMATION DURING SURGICAL TREATMENT OF PROXIMAL GASTRIC AND  ESOPHAGOGASTRIC CANCERS

The effectiveness of several union-like methods of esophageal-intestinal and esophagogastric anastomosis application has been studied in patients undergoing surgical treatment of proximal gastric cancer with involvement of esophagus, and in distal esophageal cancer. The treatment method was chosen individually, depending on the conditions of the patient and was not considered to be a standard. The union-like anastomosis gave positive results not only in mechanical but also in functional reliability of the anastomosis.
 

FUNCTIONAL STATUS OF THE PATIENTS WITH CHOLELITHIASIS AFTER ENDOSCOPIC CHOLECYSTECTOMY

The influence of endoscopic cholecystectomy on patient’s functional status and its changes during rehabilitation were studied in patients with cholelithiasis. The evaluation of respiratory function in combination with a 6-minute walking test and VAS and FACES scales made it possible to objectively evaluate the postoperative functional status of patients and its dynamic in the rehabilitation process. The most informative index of patency of airways is the value of general bronchial resistance.
 

MODERN ASPECTS OF COMPLEX TREATMENT OF CHRONIC OSTEOMYELITIS

Distant results and effectiveness of complex treatment of chronic osteomyelitis were studied in the long term of 5 years. Reduction of the purulent process, absence of pain, recovery of the limb function, absence of destruction foci and sequestrums on roentgenograms was considered as good treatment results. Analysis of the long term treatment results showed the advantages of complex patient treatment in specialized hospital departments.
 

TREATMENT OF WOMEN WITH TIMELY AND ANTICIPATORY MENOPAUSE

Evaluation of life quality in patients with premature ovarian insufficiency and hypoandrogenic condition after 12 months of hormonal replacement therapy was performed. The continuous therapy with estradiol valerate and medproxiprogesterone acetate increased the estradiol and testosterone levels, decreased the follicle-stimulating hormone and luteinizing hormone levels, normalized the Kupperman index and improved the life quality. Replacement hormonal therapy in females with premature ovarian insufficiency made it possible to maintain their physical and psychological health.
 

REASONS OF HYPERPLASTIC PROCESSES OF THE FEMALE REPRODUCTIVE SYSTEM

The immune system condition was studied in women infected by sexually transmitted diseases with hyperplastic processes of the reproductive system. A high percentage of clamydia and ureaplasma (58,1%) was found in patients with hyperplastic processes of the reproductive system, including operative material biopsies (24,4%). In patients with hyperplastic processes of the reproductive system a decrease of phagocytosis and bactericidal activity was shown which along with a decrease of potential ability of the CD3+ lymphocyte to synthesize cytokines reflected the depressed systemic immunity in women.
 

THE POSSIBILITIES OF CORRECTION OF ENDOGENOUS INTOXICATION IN CHRONIC RECURRENT SALPINGOOPHORITIS

The process of active forms of oxygen generation in blood was studied in patients with chronic salpingoophoritis using a luminal-dependant chemiluminescence registration method. The state of endogenous intoxication was also monitored in the group. Subgroups with low and high levels of oxygen radicals were determined. Plasmapheresis was shown to be effective for normalization of free radical oxidation, for reduction of endotoxicosis symptoms and for improving of treatment results.
 

EXTREMELY-HIGH-FREQUENCY THERAPY OF CHRONIC INFLAMMATION OF UTERINE APPENDAGES

The influence of millimeter range electromagnetic emission on immunological disorders and hormonal and cytokine homeostasis was studied in 105 women in the ages of 17-47 years with chronic inflammation of the uterine appendages. 77 women received extremely-high frequency therapy daily for 30 minutes beginning from the 5-7th day of the menstrual cycle, 10-20 times in total. It was shown that the usage of extremely-high frequency therapy has an obvious therapeutic effect on the immune status and hormonal and cytokine homeostasis in females.
 

THE PECULIARITIES OF PHYSICAL, NEUROPSYCHOLOGICAL DEVELOPMENT AND NEURO-ENDOCRINE REGULATION OF ORPHANAGE CHILDREN WITH PERINATALLESIONS OF THE CENTRAL NERVOUS SYSTEM

A comparative study of physical and neuropsychological development and  neuroendocrinological regulation was carried out on 160 orphanage children and on 60 children living in families. It was shown that unsatisfactory physical development, short height, body weight deficit and combinations of these factors is seen more frequently in orphanage children than in family children along with a complex delay of neuropsychological development, a significantly endocrinological disorders.
 

THE HEALTH OF MOTHERS LIVING IN VILLAGE FAMILIES OF DIFFERENT TYPES

Health status of 1577 of mothers living in village families of different types was analyzed. Significant differences were found between the health conditions of mothers from full, complicated and not-full families. Morbidity rate of diseases leading as courses of mortality was different in all three groups of mothers.
 

PRIORITY PROBLEMS OF CHILDREN HEALTH CARE IN REPUBLIC OF BURYATIA

Mortality rate of newborns were analyzed up to age of one year. It was shown that decrease of mortality at early neonatal and perinatal periods is a major direction for reduction of children morbidity, disability and mortality.
 

UNSPECIFIC IMMUNOLOGICAL RESISTANCE OF CHILDREN WHOSE MOTHERS WORK IN HOTHOUSES

The working conditions of female hothouse workers along with health and indices of unspecific immunological resistance of their children were studied. The main and control groups were formed by «copy-pair» method and consisted of 60 children in the ages 4-6 each. A high incidence of allergic morbidity and a decrease of unspecific immunological resistance indices were seen in children of female hothouse workers compared with the control group.
 

THE PREVALENCE OF THYROID DISEASES ON TERRITORIES POLLUTED BY POLYCHLORIDE CARBOHYDRATES

The distribution of thyroid gland morbidity was studied in the territory contaminated by polychlorinated carbohydrates. It was suggested that high prevalence of iodine-deficiency and goiter depends not only from endemic conditions of the territory, but also due to the presence of ecotoxicants, including polychlorinated carbohydrates.
 

THE INFLUENCE OF ANTHROPOGENIC FACTORS IN THE TIRE-PRODUCING INDUSTRY ON HUMAN VISCERAL ORGANS DEVELOPMENT IN THE FIRST TRIMESTER OF PREGNANCY

The influence of anthropogenic factors on visceral organs development in human embryos and fetuses was studied on mothers working in the tire-producing industry during the fist trimester of their pregnancy. Samples of liver and kidney tissues of human embryos and fetuses from 7 to 12 month of intrauterine development were used for studying. Abnormalities of visceral organs development of human embryos and fetuses were found. It was concluded that chemical factors of the tire-producing industry have an influence on the mother’s organism which causes certain abnormal shifts and a negative effect on human intrauterine development.
 

OPTIMIZATION OF MEDICAL ASSISTANCE TO FEMALE OF THE REPRODUCTIVE AGE AND PREGNANT WOMEN, LIVING IN AN INDUSTRIAL CENTER

The most threatening and unfavourable tendencies concerning female health were studied. Evaluating the condition of female health and finding objective criteria which allow to quantify the amount of people needing regular medical check-ups is very important for realization of the National Project «Health» and a Federal Program «Dispanserization».
 

STEP-BY-STEP DIAGNOSIS AND THERAPY OF SOMATOPHORIC AUTONOMIC DYSFUNCTION OF THE CARDIOVASCULAR SYSTEM IN ADOLESCENTS

Developed were algorithms of step-by-step diagnosis of somatophoric autonomic dysfunction of the cardiovascular system in adolescents and algorithms of complex stepby-step  clinical-psychological approaches to treatment considering clinical-psychological peculiarities of the patients and social-psychological factors. The proposed algorithms of step-by-step diagnosis and therapy of somatophoric autonomic dysfunction of the cardiovascular system in adolescents made it possible to increase the effectiveness of diagnostic and therapeutic  measures.
 

HOSPITALIZED MORBIDITY OF UROLITHIASIS OF THE ADULT POPULATION OF KAZAN CITY

The statistical analysis of hospitalized morbidity of the urban population with urolithiasis in 1998-2004 showed that the male morbidity is higher than that of female. In all age groups there was a tendency for elevation of the hospitalized morbidity.
 

A NEW METHOD OF GAS-LIQUID SANATION OF THE ABDOMINAL CAVITY IN ACUTE DIFFUSE PURULENT PERITONITIS

A new method of prolonged sanation of the abdominal cavity in acute diffuse purulent peritonitis has been developed which consists of continuous counter gas-liquid current which creates turbulent motion. This method was used in 52 patients of the main group. In 66 patients of the control group the abdominal cavity was drained by applying a prolonged decompressive laparostoma in between relaparosanation. All patients were in the toxic or terminal stage of peritonitis. The mortality was shown to be 12% lower in the main group. The proposed method was most effective in the toxic stage of peritonitis (the mortality in the main group was more than twice less) and practically had no effect on the outcome of peritonitis of the terminal stage.


EVALUATION OF THE BRAIN BLOOD FLOW BY COMPUTER-TOMOGRAPHY PERFUSION IN PATIENTS WITH ACUTE ISCHEMIC STROKES

The sensitivity and significance of computer tomography with brain blood flow investigation technique in detection of ischemic lesion foci in the brain stem has been studied. High diagnostic effectiveness of computer tomographical perfusion in diagnosing and controlling treatment effectiveness of the acute cerebral stroke was demonstrated.
