THE DEPENDENCY OF DISTRIBUTION OF CARDIOVASCULAR RISK FACTORS IN WOMEN OF REPRODUCTIVE AGE ON THE TYPE OF LEFT VENTRICLE REMODELING

106 women (21- 46 y.o.) were used in the study for the evaluation of the dependency of cardiovascular risk factors on type of left ventricle remodeling. Women with high normal blood pressure, arterial hypertension and mixture of both had usually normal geometry of left ventricle. Concentric type of left ventricle hypertrophy was associated with significantly higher figures of blood pressure, pulse pressure, index of body weight, and significantly higher percentage of patients with overweight. It was established that except modification of the life style, for prophylaxis of arterial hypertension in women of reproductive age it is very important to prevent gestation hypertension.


THE ROLE OF COMBINED METHODS OF STRESS-ECHOCARDIOGRAPHY IN DIAGNOSIS OF ATHEROSCLEROTIC OBLITERATION OF CORONARY ARTERIES

The effectiveness of stress-echocardography combined with dipirydamole (0,84 mg/kg) and submaximal bicycle test, transesophageal atrial stimulation (160 imp/min) with dipirydamole (0,84 mg/kg) or dobutamine (5- 40 mcg/min) to diagnose coronary occlusion were evaluated. 317 patients (275 men and 42 women) with suspected ishemic heart disease were used in the study. The diagnostic criteria for combined stress-echocardography methods was established. For diagnostic value such combined stress-echocardography was significantly better than standard stress-cardography method and stress-echocardography method with mono-agents.


EFFECTS OF BETA-BLOCKERS ON THE FREQUENCY OF CORONARY COMPLICATIONS DURING CAROTID ENDARTERECTOMIA

The monitoring procedure for reduction of frequency and severity of early post-operative coronary compli-cations was evaluated in 267 patients to whom the carotid endarterectomia was indicated. Inclusion of beta-blockers in pre-operational preparation significantly reduced the risk of ischemia and myocardial infarction on the first day after the operation. The frequency of early post-operative complications decreased by 28%.


THE EFFICACY OF BETA-BLOCKERS IN HYPERTENSIVE OVERWEIGHT PATIENTS

The possible negative effects of antihypertensive therapy on lipid and carbohydrate metabolism in hypertensive overweight patients have been studied. The significant reduction of systolic and diastolic blood pressure was achieved. It was found that in overweight patients the antihypertensive therapy (metoprolol and lizinopril) did not significantly affect lipid and carbohydrate metabolism.


THE EXPERIENCE OF VALVE PROSTHETIC OPERATION IN CHILDREN AND ADOLESCENTS

The early and late results of prosthetic valve operation in 27 patients (6- 18 y.o.) were evaluated. The reasons for operation included congenital heart malformations, bacterial endocarditis and rheumatism. Prosthetic operation on aortic valves was made in 15 patients, mitral valves - in 7, tricuspidal valves - in 2, and three patients had mixture valve prosthetic operation. Hospital mortality was 7,4%. Late post-operative period was monitored in 21 patients from 6 months to 14 years. It is concluded that in case of severe morphological changes the prosthetic valve operation is a method of choice in children and youngsters.


THE EVALUATION OF CLINICAL-HAEMODYNAMIC EFFICACY OF NEBIVOL AND TRIMETAZIDINE IN ACUTE CORONARY SYNDROME WITHOUT ST-ELEVATION IN ECG

Clinical and circulatory effects of nebivolol and its combination with trimetazidine were evaluated in 110 patients with acute coronary syndrome without ST segment elevation belongning to the group of intermediate risk (according to ACC/AHA classification). The use of these medicines improves management of the patients with acute coronary syndrome of the group of intermediate risk. The combi-nation treatment with these preparations resulted in the speedy regression of clinical symptoms of acute coronary syndrome and improvement of cardio-circulatory indices as assessed by ultrasound investigation. Trimetazidine increases the effect of nebivolol.


THE ROLE OF CONNECTIVE TISSUE SYSTEMIC DISORDERS IN THE GENESIS OF VARICOSE DILATION OF SMALL PELVIS' VEINS IN ADOLESCENTS

The study was carried out on 150 adolescents. In girls with varicose dilation of small pelvis veins the signs of connective tissue dysplasia were clinically determined. The increase in excretion of connective tissue metabolites (oxyproline and glycose-aminoglicans) with urine and the increase of cialic acids' serum levels were determined as a consequences of systemic disorder of connective tissue. It was established that varicose dilation of small pelvis' veins in adolescents is the evidence of connective tissue dysplasia. The results are important for development of new approaches of prophylaxis and treatment of this disorder.


ALGORITHM OF SURGEON'S ACTIONS IN PATINETS WITH SPLEEN DAMAGE

Algorithm of surgeon's actions in patients with spleen damage has been developed and the method of spleen resection has been introduced in practice. The spleen viability in post-surgery period was confirmed with ultrasound and CT- investigation of peritoneal organs using the following parameters: shape, size, parenchymal structure, outline contour, echo-density of structure and circulation volume. Ultrasound investigation and computer tomography represent objective methodology of diagnostics of spleen damage and post-surgery complications.


INTRAOPERATION PROPHYLAXIS OF PYO-INFLAMMATORY COMPLICATIONS OF WOUNDS

With the aim of increasing effectiveness of apparatus surgical wounds' correction by means of evenly distributed tension of tissues without local circulation impairment the compensating device has been developed. 36 patients with acute surgical problems of abdomen were enrolled. In 11 of them the wound opening was carried out with the help of the compensating device with the retraction force of 85.2 H and higher. In 9 if these patients the wounds were complicated with suppuration. In the rest 25 patients in which the applied retraction force was 49,9-70,5H the wounds healed without complications by primary tension. The use of the compensating device together with traditional prophylactic procedures increases the efficiency of apparatus correction of surgical wounds by means of evenly distributed tension of tissues without local circulation impairment and thus preventing wounds' complications.


CLINICAL-EPIDEMIOLOGICAL CHARACTERISTICS OF CHILDHOOD EPILEPSY IN THE CITY OF KAZAN

Clinical-statistical study of childhood epilepsy has been carried out which allowed determination of incidence and prevalence of epilepsy in children in the city of Kazan in the years of 1998-2002. The most prevalent clinical forms, age periods for the dåbut of the disease and the frequency of epileptic fits were described.


THE VERTEBRO-SPINAL INJURY: EVALUATION OF SPINAL TRANSDUCTION FUNCTION BY TRANSCRANIAL MAGNET STIMULATION METHOD

39 patients with the vertebro-spinal injury on ThVIII-LIII level and 7 healthy volunteers were used in the study. All patients had a transpedicular spondilides operation. Under magnet stimulation of movement zone of the cortex, before and after operation, the threshold, maximal amplitude and latent period of induced motor potentials of m. abductor hallucis were studied, as well as the time of motor trasnduction. The transpedicular spondilides operation cause positive effect on rehabilitation of the patients after vertebro-spinal injury.
