TO CONCEPT OF ORGANOPROTECTIVE SURGERY OF PEPTIC ULCERThe new principle of creating the organoprotective operation is suggested. The essence of the latter is in differential approach to all basic functional structures of the stomach, their adequate correction with subsequent restoration into polyfunctional organ participating in the system of regulating digestion homeostasis. The main criterion of the state of regulating systems is vitality of mucous membrane of the stomach which trophicity depends on summary effects of reverse connections. The new intervention approuch reproduces the conditions close to the natural state of organism to realize selfregulating mechanisms of digestion homeostasis and has no negative effect on trophicity of mucous membrane of the stomach. MEMBRANE DISORDERS IN DIFFERENT VARIANTS OF THE COURSE OF PEPTIC ULCERThe rate of Na+-Li+-countertransport in patients with peptic ulcer in different variants of course of this disease is studied. It is established that one of the factors determining heterogeneity of ulcers of the gastroduodenal zone is the rate of Na+-Li+-coun-tertransport in erythrocyte membrane. Peptic ulcers of the stomach and duodenum differ in membrane variations aspect. The risk factor of syntropy of arterial hypertension and peptic ulcer is the rate value of Na+-Li+-countertransport higher than 348 mmol/Li/1Er/h. CLINICAL EFFICIENCY OF THE USE OF LASER THERAPY IN COMBINED TREATMENT OF PATIENTS WITH DUODENAL PEPTIC ULCERThe dynamics of the state of microcirculation, permeability of cellular membranes by rate indices of sodium-lithium countertransport in erythrocyte membrane, the content of microelements in blood serum of patients with peptic ulcer of duodenum including laser therapy in stardard medicamental treatment is studied. Laser irradiation is used by two methods: intracavitory endoscopic irradiation of the ulcer and noninvasive irradiation through abdominal wall, irradiation of definite areas of the stomach and duodenum. As a result quicker reverse development of clinical symptoms of the disease, positive dynamics of laboratory - instrumental indices, normalization of the content of microelements in blood serum, decrease of membrane permeability by indices of sodium - lithium countertransport in erythrocyte membrane, improvement of microcirculation in intravascular part are obseverved. DEVELOPMENT OF DUODENOGASTRAL REFLUX DEPENDING ON THE TYPE OF SURGICAL TREATMENT OF PYLORODUODENAL ULCERSThe results of surgical treatment of 87 patients with complicated pyloroduodenal ulcers depending on the type of surgical treatment are analysed. The methods of determination of bile acids and specific electric resistance of stomach contents were used to determine the duodenogastral reflux. The treatment results show that the given parameters are correlated and depend on gravity of duodenogastral reflux. Duodenogastral reflux in patients in postoperative period after Bilrot I stomach resection is found by 1,5 times more rarely than after Bilrot II stomach resection. CURRENT APPROACHES TO THE TREATMENT OF PATIENTS WITH GALACTOPHORITEThe efficiency of the use of three scemes of combined treatment of 280 patients with galactophorite: action on bacterial flora, pathological secretion and affected metabolism of mammory gland tissue is determined. They proved to be highly effective and are recommended for introduction into clinical practice. Revealing of bacterial pathogene in secretory mastopathy and purposeful antibioticotherapy as well as lavage of ducts increase the efficiency of hormonal treatment of patients with mastopathy. COMBINED DIAGNOSIS OF OSTEAL METASTASES OF LUNG CANCERThe algorithm of irradiation methods of examining lung cancer metastases in bone including osteoscyn-tigraphy, polypositional roentgenography and roentgen computer tomography is developed. Among 234 patients with lung cancer the metastases are revealed in 51 (21%) patients. In 37,2% of patients osteal metastases were asymptomatical. The resolution of the applied methods of irradiation diagnosis is specified. IMPORTANCE OF ESTIMATION OF LIFE QUALITY IN PATIENTS WITH CHRONIC HEART FAILURELife quality is the integral criterion of effect of chronic heart failure on life. Life quality was studied by the Minnesota questionnaire in patients with various degree of functional disorders and etiologic variant of, chronic heart failure. Life quality criteria and clinical symptoms by the V.Yu. Marev scale were compared. A total of 21 patients with chronic heart failure in the presence of arterial hypertension and combined with ischemic heart disease of I, II, III functional class were tested. The data on the importance of determination of life quality in these patients, its decrease as the severity of the disease increases, on the inportance of clinical state estimation of patients by the score scale are obtained, the basic reasons of the life quality decrease of these patients are revealed. RESTLESS LEGS SYNDROMEFor the first time the restless legs syndrome pathogenesis is considered from positions of systemic pathology of nervous system- discoordination of motion construction program in central nervous system and activity of peripheric executors in conditions of latent myofacial trigger point of musculus gastrocnemius. Actual peripherics factor - in genesis of restless legs syndrome is the starting and supporting link. Pathologic changes of the lumbar part of vertebral column and sacroiliac articulations play a mediatory role in modulation of increased activity of spinal-reflex excitability and in reduction of suprasigment inhibitory mechanisms by formation of generators of pathologically increased excitement in rostral parts of truncus cerebri on the level of synergies of motion construction. PERSONALITY FUNCTIONS OF MENTALLY ILL PERSONS ON FORCED TREATMENTPsychological examination of mentally ill patients, who have committed especially dangerous actions being on forced treatment with intensive observation is carried out. For this purpose the Ammon Test Structure I (Ich-structure Test von Ammon) is used. It is established that the applied instrument of psychological examination (ISTA) makes it possible to estimate personality functions reflecting pathological peculiari-ties including those connected with aggressive behaviour of mentally ill persons. By more or less stable personality functions measured by ISTA the short-Term or long-term predictor of especially dangerous actions in mentally ill persons can be determined. CLINICAL PECULIARITIES OF THE COURSE OF HEMORRHAGIC FEVER WITH RENAL SYNDROME IN WORKERS OF OIL AND CHEMICAL INDUSTRYThe clinicolaboratory peculiarities of the course of hemorrhagic fever with renal syndrome in 90 males working at oil and chemical plants of Nizhnekamsk depending on age and length of work and in 129 patients never working in this industry are studied. It was found that among the workers of oil and chemical industry the systolic blood pressure was lower the diarrhea was wore opten. Low absolute number of lymphocytes and higher indices of leukocyte index of intoxication, circulating immune complexes and NST-test, lower IgA level in serum in the early reconvalescence period are revealed. USE OF CYCLOVIR-SEDICO IN PREGNANTS WITH RELAPSING FORMS OF UROGENITAL HERPETIC INFECTIONThe efficiency of new drug of the firm "Sedico" cyclovir in women with relapsing herpetic and cytomegaloviral infections during pregnancy is estimated. It is established that the drug provides reliable decrease of activity of viral infection in blood of pregnants not only with simple herpes virus but with mixed infection variant. Though the drug has no direct positive effect on cytomegaloviral infection the combined therapy including cyclovir decreases its activity in pregnants. CHOICE OF CONTRACEPTION METHOD AFTER CESAREAN SECTIONThe state of endometrium and of uterine scar in remote terms after operation is examined by ultrasound transvaginal scanning, sonocontrast and diagnostic gastroscopy in order to determine optimum methods of contraception in 65 women after cesarean section. The use of intrauterine contraception is more advisable in 5-6 months after operation. Manifestation of significant defects of myometrium in the scar area should be considered as relative contraindication to introduction of intrauterine contraceptive. MUTAGENOUS POTENTIAL OF AIR AND WATER IN INDUSTRIAL CITYMutagenous activity of pollution of air and water in industrial city is studied. The level of mutagenous activity of air on the territory of buildings in the polluted region is higher than in the relatively clean region. The comparative analysis of water from water-pipe, artesian and filtered water showed the presense of mutagenous effect only in water from water-pipe in Nizhnekamsk. Artezian and filtered water do not have these properties. Boiling of water has no effect on the level of mutagenous activity. The suggested methods of determining mutagenous activity can be used as biotest to estimate the qualitative composition of air and water. MīDERN MEDICO-SOCIAL EXAMINATION AND REHABILITATION IN OCCUPATIONAL DISEASESAt present in Tatarstan Republic 1558 patients with occupational diseases receive insurance payments on which 22423,5 roubles were spent. As many as 75% of recipients are male, 20,9% of the total number of recipients continue to work, more than half of them are persons aged 40-54. The analysis of age structure of non-working recipients showed that 80% of persons are older than 60, and 0,3% are younger than 30. The degree of disability is higher in non-working recipients. The volume of insurance payments to working and non-working recipients is 25,9% and 74,1%, respectively. MORBIDITY OF PRIMARY TUMORS OF CENTRAL NERVOUES SYSTEM OF WORKERS IN VARIOUS ECONOMY BRANCHESThe comparative analysis of morbidity of primary tumors of central nervous system of persons working in various economy branches is performed. Nonuniform distribution of the disease in workers of various economy branches is probably caused by their profession. It is necessary to improve primary preventive measures of oncologic diseases in such fields as transport, electric power engineering, oil, chemical and fuel industry. © 1995-2008 Kazan State University |
