ROLE OF PATHOLOGIC Q WAVE IN THE DIAGNOSIS OF Q-MYOCARDIAL INFARCTION DURING NON-INVASIVE SURFACE MAPPING OF THE HEARTUsing electrocardiographic surface mapping of the heart, authors introduced maps of Q3R (sum of R-wave and triple Q-wave amplitudes) and Qn (duration of Q-wave) based on criteria of pathologic Q-wave in myocardial infarction. Mapping was performed using 90 leads on patients with Q-anterior (62 patients), Q-posterior (55) and Q-diaphragmal (18) myocardial infarction at acute stage and in control group (32). The efficacy of Q3R and Qn mapping was shown for diagnosis of myocardial infarction, and the zones on chest surface were established where the changes were maximal at different localization of myocardial infarction. MORPHO-FUNCTIONAL CHANGES IN THE HEART AND ARTERIAL SYSTEM IN PATIENTS WITH POST-INFARCT CARDIOSCLEROSIS COMBINED WITH ARTERIAL HYPERTENSION IN DEVELOPMENT OF CHRONIC HEART FAILUREThe study was performed on 52 patients with post-infarct cardiosclerosis combined with arterial hypertension. It was found that progression of chronic heart failure leads to desadaptative heart remodelling and reactivity disorders of arterial bed. There was significant interrelationship between heart and vessel components of chronic heart failure. DYNAMICS OF CARDIOVASCULAR INDEXES AND SECONDARY HEMOSTASIS IN WOMEN WITH DISTURBANCES OF PSYCHOLOGICAL ADAPTATION AFTER CHOLECYSTECTOMYThe dynamics of cardiovascular and secondary haemostasis indices were studied in 150 women with disturbances of psychological adaptation after cholecystectomy during the period of hospital stay. Positive correlation between psychological peculiarities of the patients and indices of secondary haemostasis was revealed. It was shown that imbalance of psychological adaptation results in disturbances of cardio-vascular system and serves as one of the causes of complications after cholecystectomy. FUNCTIONAL STATE OF THE STOMACH AND DUODENUM IN PATIENTS WITH PERFORATIVE DUODENAL ULCER AT EARLY POST-OPERATIVE STAGE AFTER DIFFERENT METHODS OF VAGOTOMYResults of roentgenological observation of 170 patients surgically treated due to perforative duodenal ulcer were studied. Closure of perforative ulcer was supplemented by selective proximal vagotomy. In 88 patients (51,8%) vagotomy was performed by Holle-Hart skeletation method, while in 82 patients (48,2%) that was done by chemical denervation of lesser curvature of stomach with 30% water solution of ethanol. It was found that vagotomy by skeletation method supplemented with fundoplication by Nissen leads to higher frequency of mechanical disorder of high GI tract that ethanol vagotomy. It is concluded that vagotomy by selective chemical denervation is more functionally suitable to patients surgically treated due to perforative duodenal ulcer. AUTOANTIBODIES TO NATIVE DNA IN CHILDREN WITH ATOPIC BRONCHIAL ASTHMAThe level of anti-dsDNA antibodies in serum of 193 children with bronchial asthma and 21 healthy children was studied using method of ELISA. It was found that the level of anti-dsDNA antibodies increased with the severity of the disease. According to the correlation analysis, high level anti-DNA antibodies in serum depends only on the degree of severity of the disease but not on age, sex and season. The abzymatic activity of the antibodies, determined using the ability of enzymes to transform supercoiled DNA pBR 322 to circular and linear forms, was found in serum of some children with high level of anti-DNA antibodies. Anti-DNA antibodies from serum of patients with severe, and, especially, moderate degree of severity were characterized by highest abzymatic activity. INTERRELATIONSHIP BETWEEN PROLIFERATIVE ACTIVITIES OF STOMACH MUCOUS CELLS AND HELICOBACTER PYLORI IN PATIENTS WITH CHRONIC DISEASES OF THE STOMACHInterrelation of proliferative activity of stomach mucosa cells and H. Pylori presence was studied in 125 patients with chronic stomach diseases. In case of superficial gastritis, newly diagnosed acute and chronic peptic ulcers there were positive correlation between proliferative activity of stomach mucosa cells and degree of H. Pylori dissemination; in case of atrophic gastritis the correlation was negative. The proliferative activity of stomach mucosa cells was increased in case of chronic H.Pylori-associated diseases, but was reduced after successful eradication therapy. However in patients with chronic ulcer and atrophic disorders there were no such changes 1 month after the treatment. Patients with negative dynamics of proliferative activity and absence of adequate regeneration of stomach mucosa cells after eradication need monitoring for 6 months. CAUSE OF THE RELAPSE OF POST-OPERATIVE VENTRAL HERNIA AFTER SEVERAL METHODS OF ALLOPLASTYIn order to establish the main factors responsible for relapse of post-operative ventral hernia, results of 128 hernioplasty operation using meshed prosthesis were studied in delayed (up to 6 years) period. The frequency of relapse was 3,4 times higher in case of subcutaneous localization of prosthesis, than in case of retromuscular localization. In all cases the relapse occurred in the boarder of meshed prosthesis, in the zone of scar not covered by prosthesis. Age, overweight, size of hernia and early complications are not the factors responsible for relapse. LACTATE DEHYDROGENASE ACTIVITY IN THE BLOOD AND URINE IN PATIENTS WITH KIDNEY CANCER WITH DIFFERENT MORPHOLOGICAL STRUCTUREWith the aim to early diagnosis, lactate dehydrogenase activity in the blood and urine were studied in patients with kidney cancer with different morphological structure. More frequently the lactate dehydrogenase activity in the blood and urine were increased in case of mixed type of cancer comparing with clear-cell carcinoma. In patients with papillar neoplastic conditions the enzyme activity was also more often increased comparing with patients with malignant processes of parenchima. It is concluded that the measurement of lactate dehydrogenase activity in the blood and urine can be used for early diagnosis of some kidney neoplastic diseases, as well as to monitor the treatment efficacy. RELATIONSHIP BETWEEN POLYMORPHISM OF ACE GENE AND DEVELOPMENT OF NEUROPATHY IN PATIENTS WITH TYPE 2 DIABETES MELLITUSRelationship between insertio-deletion polymorphism of ACE gene and development of diabetic neuropathy was studied in patients with type 2 diabetes mellitus in Chuvash Republic. It was found that the presence of DD genotype and D allel of ACE gene significantly increases the risk of development and progression of diabetic neuropathy in these patients. APPLICATION OF MAGNETIC RESONANCE IMAGING IN THE DIAGNOSIS OF HASHIMOTO'S THYROIDITIS IN PATIENTS FROM THE IODINE DEFICIENT REGIONThe aim of the study was to assess the clinical usefulness of magnetic resonance imaging (MRI) of the thyroid gland in patients with Hashimoto's thyroiditis from the iodine deficient region. MRI signs and signal intensity ratios on fast spin-echo Tl- and T2-weighted images were measured in 50 euthyroid patients with different morphological forms of Hashimoto thyroiditis and in 20 patients with endemic goiter. Diagnosis was confirmed by sonography with colour Doppler, fine needle aspiration biopsy, and presence of antithyroid autoantibodies. It was found that signal intensity ratios correlate with histopatho-logic findings in the thyroid gland. MRI results can reflect histopathologic findings in the thyroid gland and help discriminate endemic goiter from Hashimoto thyroiditis and may be useful in differential diagnosis of various forms of these thyroid disorders. APPLICATION OF COLOUR DOPPLER SONOGRAPHY IN THE DIAGNOSIS OF HASHIMOTO'S THYROIDITISColour Doppler and sonographic measurements (peak flow velocity (PFV), resistive index (RI), pulsatility index (PI)) were performed in the inferior thyroid artery (ITA) in 300 patients with different morphological forms of Hashimoto thyroiditis. It was found that PFV correlate with thyroid function and activity of autoimmune aggression in the thyroid. Both RI and PI were on the normal range and did not show any correlation neither with the functional, nor with autoimmune activities. It is suggested that the definition of PFV in the ITA in order to estimate thyroid function on patients with Hashimoto's thyroiditis may be useful only in the cases without expressed autoimmune activity. FUNCTIONAL STATE OF THYROID GLAND IN PREGNANT WOMEN WITH GESTOSISThe serum levels of thyroid hormones (total and free fraction of triiodothyronine and thyroxine) and TTH were studied in 101 patients with gestosis and 36 practically healthy pregnant women. It was found that patients with gestosis much more often than healthy women had low level of T3 and/or freeT3 (28% and 8,3%, respectively). Low level of triiodothyronine was associated with more developed clinical and hemodynamic disorders reflecting course of gestosis. Changes of the thyroid status were secondary since they normalised after treatment of gestosis. The frequency of high level of freeT3 and/or freeT4 during gestosis was comparable with that of healthy pregnant women (8,9% and 11,1%, correspondingly). AN ANESTHETIC TACTIC DURING SURGICAL TREATMENT OF SEPTICO-NECROTIC COMPLICATIONS OF DIABETIC FOOT SYNDROMEMerits and demerits of conduction anesthesia were studied in 60 patients with type 2 diabetes mellitus complicated by septico-necrotic diabetic foot syndrome. Patients were divided into three groups according to the method of anesthesia performed - conductance anesthesia (15 patients), infiltrative anesthesia (26) and intravenous anesthesia (19). It was found that there was significantly higher level of decompensation of vital systems in the group of patients with intravenous anesthesia, while there was lower risk of intra- and post-operational complications in groups with local anesthesia. POSSIBILITY TO PRESCRIBE PANTOHAEMATOGEN FOR CORRECTION OF COPULATIVE AND REPRODUCTIVE FUNCTIONS IN PATIENTS WITH CHRONIC PROSTATITIS90 patients with chronic non-bacterial prostatitis (IIIB category) were treated with either 1% solution of pantohaematogen or camomile flower tea on top of basic treatment with general nitrous-silicon thermal bath. The high clinical efficacy of the introduced treatment, which included reduction of inflammatory process, recovery of sexual function, and normalization of prostate blood circulation, was found. ANTICARIOTIC EFFICACY OF KALTSEMIN IN CHILDREN WITH HIGH ACTIVE DENTAL CARIESThe influence of nutrition habits on the dental status of children 12-14 years of age has been evaluated using the following indices: CPU, TER-test (by Okushko V.R.), MKS (by Leus P.A.), GI (by Green-Vermilion), pH of the saliva. The effectiveness of a mineral-vitamin drug "Kaltsemin" was also studied. The beneficial effect of "Kaltsemin" was shown on the acid-base balance of the oral cavity and on the de- and remineralization in the enamel-saliva system in children. IODINE-DEFICIT STATES AMONG THE POPULATION OF REPUBLIC OF TATARSTANLegislative basis to solve the problem of iodine-deficit diseases and results of monitoring of iodinuria among the population of Tatarstan Republic were studied. Iodinuria monitoring was carried out in 2003 in 15 districts of Tatarstan Republic and in Kazan city. Epidemiological indices and the rate of iodin-deficiency were established and measures to improve the situation were introduced. CLINICO-SOCIAL CHARACTERISTICS OF PSYCHIC PATIENTS OF ELDERLY AGE COMMITTED SOCIALLY DANGEROUS ACTIONS S u m m a r yCombination of several factors leading to socially dangerous actions committed by psychic patients of elderly age was established. Clinico-psychological and social characteristics of 276 patients committed socially dangerous actions were compared with those of 230 psychic patients without such actions. Prophylactic measures were introduce to prevent socially dangerous actions of such cohort. PROGNOSTIC ACTIVITY AND COPING BEHAVIOR OF PATIENTS WITH ALCOHOLISM DURING REMISSION WITH SIGNS OF DYSADAPTATIONResults of clinical psycho-pathological and experimental psychological observations of patients with alcoholism during remission with signs of dysadaptation are shown. Prognostic activity and coping behavior of 87 patients were analyzed in a special manner. In alcohol-dependant patients an imperfection of anticipation (prognostic) activity, a coexistence of structured and non-structured coping strategies and tensity of psychological shielding was found. The peculiarities shown in the study can unfavorably effect the adaptive abilities of patients and cause progression of the disease. PROBLEMS IN TRAINING HEADS OF HEALTH CARE DEPARTMENT FOR RURAL REGIONS AND APPROACHES TO SOLVING THEMSuggestions of health-care organizers to improve manager manpower training were studied. 76 health-care managers of Kirov city and 110 of rural area were questioned while they were on the training courses in Kirov Medical Academy. The training curriculum of health manpower was changed according to the results of the questioning. MORBIDITY OF MEDICAL STAFF OF MULTIFIELD HOSPITAL BY MANIFEST FORMS OF INFECTIONSEpidemiological analysis of the morbidity of medical staff of multi-discipline hospital was carried out. Disease incidents of pharyngitis, tonsillitis, laryngotracheitis, bronchitis, pneumonia, skin and subcutaneous infections were studied. The necessity to establish new prophylactic measures to prevent medical staff from common diseases was underlined. EPIDEMIOLOGICAL IMPORTANCE OF INVASIVE FORMS OF PNEUMOCOCCAL INFECTIONIn the nosological structure of pneumococcal infections the leading position of pneumococcal pneumonia (59,06%) has been shown, while at the same time peumococcal meningitis was obviously under-diagnosed. The main risk factors are indications on different forms of ENT-disease suffered in the past 3 months such as: rhinitis (92,61%), acute otitus (82,21%), sinusitis (65,1%), tonsillitis (53,5%).
