INNOVATION IN BIOMECHANICS OF THE HEART

The experimental data showed that muscular layers making ventricle walls function in different phases and that the physiologic essence of the systole and the beginning of the diastole is identical - the contraction of muscular layers located mutually perpendiculary. These data significantly supplement the existing concept of biomechanics of the systole and revise biomechanics of the diastole of heart ventricles.


CHANGE OF CARDIOVASCULAR SISTEM INDICES IN MALE TEENAGERS WITH MITRAL VALVE PROLAPSE

Teenagers aged 15 to 17 with mitral valve prolapse were examined. The decrease of cardiovascular system capacity and its function disorder are revealed in stress ECG test. Direct connection of systolic murmur origin with regurgitation presence is confirmed. Dynamic observation of rergurgitation degree shows variation in time in 66,7% of youths the mitral regurgitation degree was the same, in each fifth youth it was increased, in 12,5% of youths it decreased. Use of the test with dosed physical load showed that in 15% of youths the mitral valve prolapse degree was increased, in each third youth the mitral regurgitation degree was increased, in 10% of youths - the symptoms appeared.


FERRICYANIDREDUCTASE AND ACETYLCHOLINESTERASE OF RED BLOOD CELLS AS MARKERS OF STRESS ECG TEST IN THE DIAGNOSIS OF STENOCARDIA IN PATIENTS AFTER MYOCARDIAL INPARCTION

Possibilities of diagnosis and differentiation of severity of the course of stable stress stenocardia in patients after myocardial infarction are studied by changes of activity of membranerelated enzymes of erythrocytes - ferricyanidreductase and acetyl-cholinesterase in early recovery period of stress ECG test. It is shown that during the stress ECG test the decrease of activity of these enzymes is dependent on severity of the course of stable stress stenocardia. Coefficients of decrease in their activity in early recovery period of the test are effective markers of ischemic response of myocardium and can be used for the diagnosis of stable stress stenocardia and the determination of its functional class.


EFFECT OF MEBICAR ON WORKING ACTIVITY OF PATIENTS WITH PSYCHIATRIC DISORDERS

Requirements on psychotropic drugs, limiting their use on the background of working activity are indicated. The original complex of methods and results of treatment of working patients with day tranquili-zer - mebicar are described. Clinical observations are confirmed by the results of experimental and psychological study on the reaction time analyser.


RESULTS OF PROLONGED MONITORING OF PULMONARY FUNCTION TEST IN PATIENTS WITH CHRONIC OBSTRUCTIVE PULMONARY DISEASE AND BRONCHIAL ASTHMA

Parameters of the flow - volume curve of forced expiration are registered during three years in 60 patients with bronchial asthma and 98 patients with chronic obstructive pulmonary disease taking treatment according to GINA and GOLD recommendations. Reliable improvement of parameters has been noted during the first six months of basic and symptomatic therapy. A decrease of bronchial permeability indices has been significantly retarded during three years. Incomplete parallelism of variations of peak expiration flow and volume of forced expiration per one second in patients with bronchial asthma should be noted. Peak expiration flow is preserving for longer time and it is considered to be the most important in dex in self-control of asthma, should be noted.


EXPERIENCE OF THE USE OF AMIXINE IN HEMORRHAGIC FEVER WITH RENAL SYNDROME IN THE EAST KAMA REGION

The efficacy of amixine in treatment of hemorrhagic fever with renal syndrome is studied. The decrease of fever duration on the first day of starting treatment, 3 fold seldom development of vomiting syndrome and reduction of vomiting syndrome duration from the disease onset 3-4 fold, as well as azotemia duration by two days from the disease onset, the decrease of maximum indices of thymol test 1, 8 fold, plasma creatinine by 21,0%, leukocytic intoxication index after the treatment with amixine 1,5 times, the growth of specific immune response 3 times in early terms of the disease are noted. The decrease of economic expenses on one patient is noted. The use of amixine in the initial period of hemorrhagic fever with renal syndrome in daily dose of 0,25 g is indicated.


PERIPHERIC BLOCKADES IN CHILDREN DURING OPERATIONS ON UPPER AND LOWER EXTREMITIES PERFORMED WITH NEUROSTIMULATOR

Combination of regional anesthesia with superficial anesthesia presents the advantages of both methods, in this case protecting children from excess narcotization and avoiding many dangerous drawbacks of general anesthesia. Appearance of neurostimulators and current anesthetics makes it possible to use widely peripheric regional blockades in children of all ages during operations on upper and lower extremities.


TOPOGRAPHOANATOMIC JUSTIFICATION OF INSERTION OF TROACHARS IN ABDOMINAL ENDOSCOPIC SURGERY

Examination of neurovascular fascicles of the anterior abdominal wall was performed. The detailed syntopy of basic superficial neurovascular fascicles of the anterior abdominal wall was studied. We recommended nonvascular zones for placing troachars. It would decrease the rate of lesions of vascular collectors of the anterior abdominal wall.


OPTIMIZATION OF THE LEVEL OF FORMATION OF BILIODIGESTIVE ANASTOMOSIS IN GASTROPANCREATO-DUODENAL RESECTION

High postoperative lethality due to incompetence of pancreatojejunal and biliodigestive anastomoses restricts its application. Case histories of 139 patients after gastropancreatoduodenal resections in the Kazan Oncological Clinic of the Ministry of Health of Tatarstan Republic were studied. Intraorgan hemodynamics of external biliferous ducts was examined. The original method of prevention of incompetence and stricture of biliodigestive anastomosis is suggested.


MANAGEMENT OF COMPLICATED FORMS OF COLON CANCER IN EMERGENCY SURGERY

Experience of the treatment of 104 patients with complicated colon cancer in emergency surgery is studied. It is established that in complicated forms of colon cancer the operative intervention should be minimal. Operations with tumor removal during the first stage should be considered most appropriate because delay will result in cancer propagation.


TREATMENT OF NEUROGENIC CONTRACTURE OF HAND FINGERS

It is established that stabilization of the main phalanx in the functional position provides normal prehensile function of middle and distal phalanges of hand fingers in neurogenic paralyses of interosseal and vermicular hand muscles. Arthrodesis of metacar-pophalangeal articulation in the functional position of the main phalanx (phalanges) of hand fingers is recommended.


PEROSSEOUS OSTEOSYNTHESIS IN THE TREATMENT OF PATIENTS WITH DISLOCATION FRACTURES OF THE HUMERUS HEAD.

CURRENT PROBLEMS OF MEDICAL OBSERVATION OF WORKERS IN UNFAVOURABLE LABOUR CONDITIONS

The analysis of the quality of periodical medical examinations shows low revealing of occupational diseases by the majority of medical institutions of Tatarstan. Occupational diseases were suspected in 4,4% of the examined workers, whereas the former index was only 0,9%. It is necessary to include specialised medical institutions to periodical medical examinations of persons working in contact with unfavourable industrial factors.
