N.A. BERNSTEIN'S IDEAS - METHODOLOGIC BASIS FOR REHABILITATION THERAPY IN NEUROLOGY AND OTHER CLINICAL DISCIPLINESIn 1929 N.A. Bernstein for the first time proposed basic ideas about motion function of human organism as a complex-organized system. Based on his ideas, intensive research is carried out currently in Kazan State Medical Academy to study etiology, pathogenesis and treatment of neurologic and other diseases. PAINFUL PARESTHESIA SYNDROME DURING MYOFASCIAL PAIN SYNDROMEDescription of clinical signs of night painful hand paresthesia in patients with myofascial pain syndrome was given. Paresthesia evaluated in connection to intensity, appearance at day- or night-time, localization, irradiation, and affection of night-sleep quality. Differentiative methods to treat paresthesia were introduced. CEREBRAL HEMODYNAMICS IN PATIENTS WITH ARTERIAL HYPERTENSIONDependence of cerebral haemodynamics on level of arterial pressure, course of disease, character of arterial pressure daily profile, presence and expression of structure disturbances of vessels and endothelial disfunction was studied in patients with arterial hypertension. The results of ultrasound examination of brain and shoulder arteries, daily monitoring of arterial pressure, endoteline-1 definition in blood are presented. It was proven that alterations of cerebral haemodynamics in patients with arterial hypertension were connected to arterial pressure level, complicated course of the disease, daily arterial pressure profile and expression of endothelial disfunction. HYPERHOMOCYSTINEMIA AND REMODELLING OA VESSELS IN PATIENTS WITH ARTERIAL HYPERTENSION110 patients with different degree of arterial hypertension and 28 control healthy volunteers were under observation. It was found that the level of homocysteine was significantly higher in patients with 2nd and 3rd degree of arterial hypertension that that of patients with 1st degree or healthy volunteers. Significant correlation was found between serum level of homocysteine and thickness of intima-media complex of carotid arteries in patients with 2nd and 3rd degree of arterial hypertension. It is suggested that homocysteine could play a direct role in formation of carotid atherosclerosis which in turn forms and keeps high arterial pressure. ELECTROMIOGRAPHIC RESEARCH OF THE FUNCTIONAL CONDITION OF CHEWING GROUP OF MUSCLAR-JOINT DISFUNCTION OF TEMPORAL-MANDIBULAR JOINTThe article analyses the results of electromiographic research of 25 patients ill with musclar-joint disfunction of temporal-mandibular joint. Significant changes of EMG picture are revealed - asymmetry of lateral-pterigoidus muscles, spontaneous activity of chewing ones, increased activity of the oral cavitily bottom muscles. RISK FACTORS FOR ARTERIAL HYPERTENSION IN CHILDREN AND TEENAGERSTo distinguish a risk group, 88 teenagers with arterial hypertension were under observation. It was found that 18% of patients belong to low risk group, 59% - to intermediate, and 23% - to high risk group for cardiovascular events. The results suggest the necessity to use a complex program to act on risk factors such us correction of body weight, giving up smoking, hypolipidemic diet. METHODS OF PROPHYLAXIS OF ASPIRATIONAL PNEUMONIA IN NEWBORN WITH ESOPHAGEAL ATRESIAApproaches to improve the methods of surgical manipulation for prophylaxis of aspirational pneumonia in newborn with esophageal atresia are described. The necessity of early intervention is underlined. LIFE-TIME CLINICO-LABORATORY DIAGNOSTIC OF HYDROPHOBIAA case of lethal hydrophobia in patient bitten by mad wolf is described. CLINICO-FUNCTIONAL RESULTS OF SPHINCTER-SAVING OPERATIONS WITH IN PATIENTS WITH RECTAL CANCERResults of 150 sphincter-saving operations were analysed, including 18 cases where newly developed method of formation of H-form caecal reservoir was performed on patients with rectal cancer. It was shown that formation of H-form caecal reservoir increases the functional results of the surgical treatment, as well as patient's quality of life without increasing the mortality. PECULIARITIES OF UTERUS CONTRACTILE ACTIVITY IN PREGNANT WOMEN WITH CHRONIC UROGENITAL INFECTIONS306 pregnant women with chronic urogenital infections were under observation during their last few weeks of pregnancy and compared with 54 healthy pregnant women. Spontaneous contractility of the uterus was registered by external four-channel hysterography. It was found that in the presence of urogenital infections mechanical activity of the uterus demonstrates a pathologic character, and has some specific differences during different infections. NON-GAS LAPAROSCOPY IN GYNECOLOGY74 women were surgically treated using laparoscopic isopneumatic regimen. It was found that this method has technical advantages and leads to decrease of postoperative pain comparing with conventional methods. THE PECULIARITIES OF FEMALE HORMONAL STATUS IN PATIENTS WITH BREAST CANCER COMBINED WITH REMOTE METASTASISThe relationship between the quantity of several blood hormones and the presence of remote metastasis in 112 patients with breast cancer has been studied. The hormonal profile of 92 patients was studied before the operation on the mammary gland, while in 20 patients it was studied after establishing the presence of remote metastasis. Comparing the 2 groups of patients with and without remote metastasis established was the fact that in patients with remote metastasis the level of prolactin (p=0,001), lutropin (p=0,009) in the blood serum is higher, while the level of progesterone (0,021), estradiol (p=0,001) and testosterone is lower than in the second group. Proposed is the targeted suppression of the prolatin-producing function of the pituitary gland as one of the components of hormonal therapy of breast cancer. CENTRAL HEMODYNAMIC IN PATIENTS WITH INFLAMMATORY DISEASES OF MAXILLOFACIAL REGIONThe status of cardiovascular system was studied in 157 patients with inflammatory diseases of maxillofacial region. It was shown that cardiovascular changes during inflammatory diseases of maxillofacial region reflect the general processes in patient's organism and is due to toxic influence of products of degradation in the site of inflammation. These changes are not specific for this condition but reflect the systemic response of cardiovascular system on inflammation. COLONIC RESISTANCE AND HUMORAL DEFENCE FACTORS OF ORAL MUCOSA IN PATIENTS WITH CHRONIC PARADONTOSISColonic resistance and adhesive activity of secretory opsonin were studied in patients with chronic paradontosis. Results demonstrated the informatic value opsonophagocytic defense mechanisms showing severity and dissemination of inflammatory process in parodont. SURGICAL TREATMENT OF CHRONIC PANCREATITIS AND PANCREATIC CYSTSThe results of the surgical treatment of 193 patients with complicated chronic pancreatitis in 1995-2005 were analysed. The frequency of postoperative complications came down from 8.3% in 1995 to 4.2% in 2005. It was found that postoperative complications more often happened after operations on adjacent organs, than after draining operations and resections of the pancreas. The mortality after resections of the pancreas was higher than after draining operations. The remote results after resections of the pancreas were better than after internal draining operations and much better than after externally draining operations. COMBINED RECONSTRUCTIVE THORACOPLASTY USED FOR TREATMENT OF FUNNEL-SHAPED CHEST DEFORMATIONSThe efficacy of treatment methods of funnel-shaped chest deformations was compared. 41 patients of the first group were treated by combined reconstructive thoracoplasty with complex rehabilitation therapy, while 40 patients of the second group had traditional reconstructive thoracoplasty. Good results were obtained in 88% of patients, satisfactory - in 8%, and non-satisfactory - in 5% of patients. The advantage of using computerised tomography for diagnosis and choosing the treatment methods is underlined. PLASMASPHERES IN TREATMENT OF IDIOPATHIC INFLAMMATORY MYOPATHY134 patients with dermatomyositis and polymyositis (idiopathy inflammatory myopathies) were studied to evaluate the efficacy of different efferent methods of therapy. It was found that the most effective was complex therapy with immunosuppressive drugs and plasma-exchange with cryosorption method. COLOR VISION IN SCHOOL CHILDREN LIVING IN AN INDUSTRIAL DISTRICTThe color vision of 200 school children living in the city district polluted by chemical industry was compared with that of 100 school children living in the relatively clean city district without industrial pollution. It was found that children living in the polluted district had congenital impaired color vision. The frequency of the color vision problems depends on type of pollution. DISABILITY DUE TO PROFESSIONAL DISEASES IN ULYANOVSK REGIONDisability due to professional diseases was analysed in Ulyanovsk region for the period of 1995-2004. It was found that for the last few years the disablement increases to 21.5% per year. The level of initial disablement is 1.5 times higher in Ulyanovsk region than in Russian Federation in average. These results are needed to consider for planning measures to prevent morbidity and disability due to professional diseases. MORTALITY OF POPULATION OF TATARSTAN REPUBLIC FROM TRAUMAS AND OTHER EXTERNAL FACTORSThe mortality of Tatarstan population due to traumas and other external factors was analysed for the period of 1995-2005. Every year 7000 peoples perish from traumas and other external factors in Tatarstan Republic, most of them men of work-age. Morbidity of rural population is higher than that in the cities. For the last 5 years, there is a tendency to decrease morbidity due to suicide and homicide; however the morbidity due to road accidents and intoxications has a tendency to increase. STATE REGISTER OF DIABETES INSIPIDUS IN CHILDREN AND TEENAGERSState Register of diabetes insipidus prepared by the specialists of Republican Clinical Children Hospital was analysed. The State Register has information about 301 patients under 17 with diabetes insipidus. The data of State Register were compared with those of Endocrinological Scientific Center (Moscow). It was shown by applying standardization method that in case of equalised conditions frequency of complications would be significantly higher with observation of the patients in the Republican Clinical Children Hospital. PROBLEMS OF DEVELOPMENT OF RATIONAL INFRASTRUCTURE OF HEALTH CARE SYSTEM IN THE REGIONGeneral problems of development of rational infrastructure models of health care system in the region were considered, including the model maximizing the quality of the health care system and the model minimizing the expenses. © 1995-2008 Kazan State University |
