TRANSFORMATIONS OF SYMPATHOADRENAL SYSTEM IN ELDERLY AND SENILE AGE AS RISK FACTORS OF CARDIOVASCULAR DISEASES

The phenomenon of early postnatal involutive changes of adrenergic plexuses in norm is established in studying ontogenesis of sympathic plexuses of human heart using early dissections data by neurohistochemical methods. Decrease of their density occurs after 30-year age in working myocardium and along the conductive system. In denervation of tissue according to the Kennon-Rozenblyut law, tissue sensitivity to humoral factors increases changing distinctly the concept of essence of heart sympathic regulation in old age - the immediate fast nervous regulation of activity of organs and systems as viewed from sympathic part of nervous system and its adaptational and trophic action is replaced with age by slow and less targeted humoral regulation method.


CLINICAL IMPORTANCE OF DETERMINING OF THE RATE OF NA+/Li+-COUNTERTRANSPORT IN PATIENTS WITH CHRONIC HEART FAILURE

The functional state of cell membranes - the rate of Na+/Li+-countertransport (Na+/Li+-CT) is studied in patients of both sexes with chronic heart failure, based on various heart pathology, with various functional classes (NYHA, 1964) and depending on nature of left ventricle disfunction. The analysis of incidence of separate variants of chronic heart failure in quartiles of rate values of Na+/Li+-CT showed that carriers of high rates prevail among women and carriers of low and moderate rates of Na+/Li+-C’ prevail among men. Men with arterial hypertension as an ethiologic factor of chronic heart failure more aften have low rates of Na+/Li+-C’, and men with ischemic heart disease have high gradation of functional disorders of heart activity, being carriers of high rates of Na+/Li+-C’ IV quartile.


PECULIARITIES OF THE COURSE OF ARTERIAL HYPERTENSION IN MENTALLY ILL PERSONS IN PROLONGED COMBINED THERAPY WITH PSYCHOTROPIC MEDICATIONS

Peculiarities of the course of arterial hypertension are studied in 158 mentally ill patients during prolonged combined treatment with psychotropic drugs. The efficiency of combined treatment of arterial hypertension depending on combination, daily doses and psychotropic therapy duration is estimated. It is established that mentally ill persons have high incidence of modified risk factors, requiring additional correction of factors impairing arterial hypertension prediction, despite the positive dynamics of arterial hypertension level in the majority of patients. The most favourable course of arterial hypertension was observed in persons getting combined psychotropic therapy with two or three drugs, rational use of which in the majority of cases made it possible to decrease doses of hypotensive drugs or abandon them.


PERMEABILITY OF THE INTERNAL ILIAC ARTERY AT REMOTE TERMS AFTER AORTOFEMORAL RECONSTUCTIONS

Permeability of the basic collateral collector in occlusions in this zone- the internal iliac artery is studied to estimate the state of collateral arteries at remote terms after aortofemoral reconstructions. The study was carried out on the basis of analysis of angiograms of 63 patients. Three-year cumulative permeability of the internal iliac artery was 47,2+6,2%, five-year permeability was 21,8+6,9%. The dependence of remote permeability of the internal iliac artery on surgical technique is revealed - the distal anastomosis "end to end" provides higher permeability in the external iliac artery.


SCARRY DEFORMATION OF DUODENUM IN SURGERY OF PEPTIC ULCER

Definite laws of development of scarry deformation are found on the basis of intraoperational examinations in 122 patients with peptic ulcer of duodenum, complicated by penetration of ulcer in the pancreas head. Deformation formation mechanism revealed in gastroduodenal zone showed that "difficult to ablate" postbulbar ulcer is ratber relative and in case of use of developed technical methods allows a surgeon to carry out the Bilrot I stomach resection in most cases.


NEW APPROACHES IN THE TREATMENT OF GENERALIZED PERITONITIS

The results of the treatment of 288 patients with acute generalized peritonitis by various surgical methods are analyzed. It is established that laparoscopic sanations of abdominal cavity can be successfully used in combined treatment of acute generalized peritonitis as highly effective method of local therapy.


CEREBRAL CIRCULATION DISORDER: ORGANIZATION AND PROSPECTS OF DEVELOPMENT OF SERVICE TO PATIENTS WITH CEREBRAL STROKE IN TATARSTAN REPUBLIC

Indices of morbidity, disability and mortality caused by cerebrovascular diseases, in particular, cerebral stroke are analysed. Obvious tendency to their rise in population of adults in Russia as a whole as well as in Tatarstan Republic is established. Negative dynamics of these indices is determined not only by social and economic reasons but by the existing system of medical service. Emergency and differentiated medical service to significant number of patients with cerebral circulation disorders is not timely provided, is not sufficiently performed, in this case its efficiency is reduced to a minimum due to drawbacks before hospital and in first hours on hospital stages. Importance of primary and second prevention is stressed.


CHANGES OF HEMOCOAGULATION AND FIBRINOLYS IS IN PATIENTS WITH BREAST CANCER DURING TREATMENT

The effect of chemotherapy on hemostasis parameters is studied in patients with breast cancer after radiation therapy and mastectomy. The increase of blood coagulation potential and decrease of fibrinolitic system activity were noted in all patients before chemotherapy. Reliable steps relative to the state before chemotherapy were not established during chemotherapy in patients with breast cancer by electrocaulogram data. This serves as an indication of high risk of tromboses development in patients. Prolonged chronometric hypercoagulation, antitrombin III deficit and high values of fibrinomonomer complexes in all examined persons show the presence of latent intravascular coagulation syndrome during polychemotherapy.


REPRODUCTION OF HEALTH OF POPULATION AS A SUBJECT OF STUDYING MEDICAL DEMOGRAPHY

The problem of reproduction of health of population which is considered to mean the process of continuous renewal of generation vitality is viewed. Reproduction of health of population should present itself as quantitative index of health state of population. Being a subject of studying medical demography this index makes it possible to estimate general direction of changing health of generation.


PRESENTATIONS ON ETHICAL AND LEGAL PROBLEMS OF HIV-INFECTION IN MEDICAL STUDENTS

Special study, performed among students showed that on the background of rise of legal consciousness in current medical students the level of their ethical and legal knowledge can not be considered satisfactory. Relations of medical students with HIV-infected patient are characterized by peculiar dualism. On the one hand he is ready to help a patient, on the other hand he has antihumane position including organization of special reservation for patients with AIDS. In educational process of current medical institutes it is necessary to find and introduce new methods and forms of education of humanism and professional duty in future physicians.


MINERAL COMPOSITION OF WATER AND CONTENT OF MACRO- AND MICROELEMENTS IN SALIVA OF CHILDREN

The correlation between mineral composition of water and content of macro- and microelements in saliva of children is estimated. Conditions of water supply and qualitative composition of water of two large residential districts of Kazan, which have significant differences in chemical composition of water are studied. The correlative analysis showed the presence of dependence between content of metals in saliva and chemical composition of water. Studies suggest the presence of disbalance of macro- and microelement homeostasis in children using water of unfavourable mineral composition.


STANDARTS OF PHYSICAL DEVELOPMENT OF STUDENTS

According to anthropometric data the standards on seven indices of morphofunctional development of students were developed, for estimation of physical development of students in Tatarstan Republic. Comparison of indices of morphofunctional development in students shows the dependence of dynamics of these indices on physical training program. Indices of physical development of students can be included in social and hygienic monitoring as an objective criterion of health, sanitary and epidemilogic well-being of students of Tatarstan Republic.
