STRESS-ECHOCARDIOGRAPHY WITH DIPYRIDAMOLE AND TRANSESOPHAGEAL ELECTROSTIMULATION OF AôRIá IN DIAGNOSIS OF ISCHEMIC HEART DISEASE

A.Yu. Vasilyev, N.N. Mikheev
Sensitivity and specificity of combined stress-echocardiography with dipyridamole in high dose and transesophageal electrostimulation of atria in diagnosis and prevalence of stenosing lesions of coronary arteries in ischemic heart disease in a group of patients not capable of performing physical exercise, are determined. 49 persons were examined. Sensitivity and specificity of combined stress-echocardiography are higher comparing with stress-electrocardiography. Nature of segmental disturbances of contractility on load peak in combined stress-echocardiography makes it possible to determine localization of stenosing atherosclerosis of coronary arteries with sensitivity up to 97,3% and to diagnose multivascular lesion with sensitivity up to 100% and specificity up to 100%.

PECULIARITIES OF HEART RATE AND ITS VARIABILITY IN BOYS OF JUNIOR SCHOOL AGE DEPENDING ON PREVAILING SLOW WAVE VIBRATIONS OF CARDIAC RHYTHM

A.R. Sabiryanov, E.S. Sabiryanova, I.G. Petrivanov
Heart rate and its slow wave variability depending on prevalence of vibrations power in spectrum ranges in boys of junior school age are studied. It is found that in healthy boys in junior school age depending on heart rate the variability of cardiac rhythm can prevail in any slow wave spectrum range. Results of the study showed that in spite of origin difference of the most low rate and high rate variability, heart rate is determined by high centers of vegetative regulation causing negative chronotropic effect on cardiac rhythm.

ON PROBLEM OF CLINICAL CLASSIFICATION OF ACUTE ADHESIVE ILEUS

V.V. Plechev, S.A. Pashkov
Clinical classification of acute adhesive ileus is suggested on the basis of treatment of more than two thousands of patients. Built on principles of simplicity of perception, easiness of memorizing, reflection of tactic approach to diagnosis and treatment, possibility of data processing and based on morphofunctional changes, the classification reflects clinical manifestations of the disease and improves the treatment of patients with acute adhesive ileus.

PREVALENCE AND PREDICTION OF RISK FACTORS OF REPRODUCTIVE DISORDåRS OF WOMEN

A.G. Ivanov
Prevalence of sterility in Tver is studied, statistically reliable medicobiological and social and hygienic risk factors determining development of this pathology are revealed. Statistically reliable risk factors of primary and second inferility are revealed and prognostic tables of individual risk of their development are built. Use of prognostic tables abong with medical examinations helps to reveal patients needing dispensary observation and treatment.

DIFFERENTIAL DIAGNOSIS OF HEADACHE IN PATIENTS WITH MENTAL DISORDERS

G.A. Ivanichev, A.V. Golenkov, F.V. Orlov
Problems of differential diagnosis of headache in persons with mental disorders are studied. Clinical features of cephalalgic syndrome were analysed in the framework of neurotic and somatophormic, schizophrenic, affective, organic mental disorders. It is shown that diagnosis of headache in persons with mental disorders does not only mean diagnosing of mental disease or cephalalgia but as well includes determining headache as psychopathologic formation. Differential diagnosis stages of headache on the basis of complex clinicopsychologic method are developed.

EFFECT OF NATURE OF SENSIBILIZATION ON SEVERITY OF ATOPIC DERMATITIS IN CHILDREN OF EARLY AGE GROUP

N.B. Migacheva, A.V. Zhestkov, T.I. Kaganova
Spectrum of causal allergens in children of early age with atopic dermatitis and effect of nature of sensibillization to various exoallergens on severity of the disease are studied. It is established that in spectrum of causal allergens, food, domestic and epidermal allergens prevail. Statistically significant relation between severity of atopic dermatitis and sensibilization to fungous, pollen allergens, beef, rye, potatoes, tick of dust and canine epidermis is revealed. Determination of spectrum of causal allergens in children of early age with atopic dermatitis is of major importance for prediction of severity of the disease as well as for optimization of measures directed to prophylaxis of this disease in children of risk group.

PECULIARITIES OF BLOOD FLOW IN HUMAN BRAIN BASE ARTERIES IN EARLY ONTOGENESIS

V.D. Gruzdev, A.L. Azin, A.L.Zefirov
Hemodynamics in villisy's circle arteries is studied by linear blood flow velocity at the age aspect and peculiarities of functioning of brain base arteries system in children during first year of life are estimated by transcranial dopplerography depending on delivery course and fetus position. It is shown that brain base arteries system of children during first year of life functions in so-called "overlimit manner" expressed in very high linear blood flow velocities in all parts and in arising sonographic signs of turbulence flows with increased peripheral resistance. In pathologic delivery course blood flow in vertebral arteries tends to decrease and blood supply deficit is more pronounced after delivery with II head fetus position.

HYGIENIC ESTIMATION OF SOIL IN KAZAN

N.V. Stepanova, R.Ya. Khamitova
Hygienic estimation of soil pollution by heavy metals of various regions in the large city using current methodical approaches (comparison with con-centrations, background values, obtained by three different approaches and home regulations for total and slip forms) is hÅld. It is established that soil of old industrial regions of the city is severely polluted by some metals and by the sum index it is dangerous for health of population.

INFORMATION ON RIGHTS OF PAôIENTS IN DRUG ADDICTS

R.G. Sadykova
The level of information on rights of patients in drug addicts is shown. It proved to be quite high. Most of drug addicts know their right to choose medical institution and doctor. The analysis of quality of medical service from the point of view of patient showed that 51,6% of respondents are satisfied by it, 36,3% of them are not quite satisfied by it and 4,8% of them are not satisfied. There are facts of violating rights on information on the suggested treatment method, on keeping medical secret.

MONITORING OF NATIONAL DRUG POLICIES IN COUNTRIES OF THE MIDDLE ASIA REGION

T.S. Nurgozhin, A.E. Gulyaev, L.E. Ziganshina
Results of the analysis of basic manifestations and signs of national drug policy in countries of the Middle Asia region are given. Its elements are present in all these countries. However in Kyrgyzstan this tendency is realized in the most full degree and availability of drugs is higher. Introduction of programs of estimating the use of drugs and monitoring their side effects within the limits of formular system can be of particular value.

ON IMPROVENT OF FINANCIAL SYSTEM OF TREATMENT AND PROPHYLACTIC INSTITUTIONS GIVING EMERGENCY TRAUMATOLOGIC CARE

I.F. Akhtyamov, S.V. Krivoshapko, G.M. Krivoshapko
Financical system of treatment and prophylactic institutions giving emergency traumatologic care is characterized. Necessity of reorganizing calculation system of state hospital cash-desks with treatment institutions is justified. Concrete suggestions are recommended to increase effective work of trauma-tologic departments and to improve quality of giving specific medical care to traumatologic patients.

PROSPECTS OF DEVELOPMENT OF ORGANIZATIONAL AND METHODICAL WORK AS THE BASIS OF MANAGEMENT OF ACTIVITY OF NET AND PERSONNEL OF TREATMENT AND PROPHYLACTIC INSTITUTIONS OF ADMINISTRATION TERRITORY

Sh.M. Vakhitov, M.V. Blokhina, L.I. Rybkin, G.A. Shamseeva, A.M. Fatykhov
Organizational and methodical work as the basis of management of activity of net and personnel of treatment and prophylactic institutions is estimated as specific sphere of marketing activity. Purposes, problems and directions of development of organizational and methodical work at present taking into account specificity of making activity plans of treatment and prophylactic institutions are shown.

NEW PLASTIC CONTAINERS FOR OBTAINING THE THERAPEUTIC DOSE OF THROMBOCYTIC CONCENTRATE FROM A SINGLE DONOR

E.P. Svedentsov, V.V. Zhuravlev, M.I. Kaminskaya, G.N. Plotnikova
KP KM 600/400/400 plastic tripled containers for thrombocytopheresis are developed. The plastic container with 600 ml of stabilized blood is placed into 750 mm glass container and is centrifuged with 880 g acceleration at +22œC during 8 min. For separation of thrombocytic concentrate from the plasma enriched by thrombocytes it is centrifuged with 3400 g acceleration, at +22œC during 15 min. Thrice-repeated thrombocytopheresis with the containers and centrifugation regimes suggested makes it possible to obtain the complete medical dose of thrombocytic concentrate equal to 4,17+0,28_1011 blood plates during 1-1,5 h from one active donor with body mass not less than 65 kg.
