EFFECT OF PRESENCE AND SEVERITY OF ARTERIAL HYPERTENSION ON TEMPORAL INDÅÕES OF VARIABILITY OF CARDIAC RYTHM

It is established that as the severity of arterial hypertension progresses, the variability of cardiac rhythm in patients with severe arterial hypertension decreases. While there is a general decrease of the effect of parasympathetic part of autonomic nervous system with the growing severity of arterial hypertension in patients with severe hypertension, this effect is most pronounced in day-time. The regulation of cardiac rhythm by temporal parameters in patients with severe arterial hypertension differs from that in normotonics and patients with mild and moderate forms. This allows considering the regulation of cardiac rhythm as a specific qualitative status requiring adequate approaches to the treatment.


PECULIARITY OF DIASTOLIC DYSFUNCTION IN PATIENTS WITH ARTERIAL HYPERTENSION WITH NORMAL LEFT VENTRICLE MASS

235 patients of 30-65 years of age were examined. The degree of left ventricle hypertrophy was evaluated by echocardiography. In patients with arterial hypertension with normal left ventricle mass, the diastolic dysfunction is the main marker of cardiac muscle lesion. Active relaxation indices are the objective and early criteria of diastolic dysfunction of left ventricle. The use of active relaxation indices in patients with arterial hypertension makes it possible to refine and objectify the disease stage.


CLINICAL PRESENTATION, PREVALENCE, PATHOMORPHOGENESIS AND OUTCOMES OF TUBERCULOSIS OF ALIMENTARY TRACT

Clinical, laboratory and instrumental parameters were analyzed in 126 patients with tuberculosis of alimentary tract for the period of 1980-2004. Morphogenesis data were studied by endoscopy with target biopsy, laparoscopic and laparotomic operative interventions with partial resection and biopsy on autopsy. In 54,0% of observations tuberculosis of alimentary tract correlated with pulmonary tuberculosis, in 24,6% of observations - with extrapulmonary tuberculosis, in 19,8% it was registered as hepatitis and casuistical cases of cholecystitis as independent, but not classified clinical forms. In 46,0% of cases two and more organs are involved in alimentary tuberculosis. Lethality in alimentary tuberculosis varied from 12,5% to 85,0%.


MODERN METHODS OF RADIATION DIAGNOSTICS OF CHRONIC PYELONEPHRITIS AND NEPHROGENIC ARTERIAL HYPERTENSION

Possibilities of modern methods of radiation diagnostics (ultrasound, digital subtractional angiography, aortography, NMR-angiography, computer tomography etc,) with account of clinical, laboratory data and renography in diagnostics of chronic pyelonephritis, parenchymatous, vasorenal arterial hypertension were studied. It was established that modified excretory urography with calculation of biometric indices of kidneys is a highly informative diagnostics method of early stage of chronic pyelonephritis. Ultrasound contrast examination makes it possible to study characteristic changes of cortical and cerebral blood flow in various stage of the disease.


MORPHOLOGY OF LOW-HORIZONTAL ELECTROSURGICAL GASTROENTEROANASTOMOSIS IN EARLY POSTOPERATIVE PERIOD IN EXPERIMENT

Experimental study of characteristics and time intervals of healing of two types of gastroenteroanasto-moses - the open low-horizontal anastomosis when the first row of sutures goes through all layers of the wall by blanket variant and the closed low-horizontal anastomosis by electrosurgical method by the method of the authors, was performed. Experimental studies were carried out on 30 mongrels with body mass of 16 to 21 kg. Algorithmic presentation of experimental biologic material included light microscopy of sections, colored with hematoxylin-eosin and by Van-Gizon. It was established that the electrosurgical method of forming gastroenteroanastomosis as compared with the open method makes it possible to minimize traumatic effect, to decrease intensity of exudative stage, inflam-mation, to accelerate reparation with reconstitution of structure by 14-th day and to restrict the process to the mucous membrane and submucous layer.


ANTITHROMBOTIC PREVENTION AFTER LAPAROSCOPIC CHOLECYSTECTOMY IN ELDERLY PATIENTS WITH EXCESS BODY MASS

Vascular-platelet and coagulation phases of hemostasis and fibrinolytic activity of blood were studied in 20 relatively healthy persons over 60 years of age with normal body mass and in 65 patients of the same age group with body mass index exceeding 25,0 kg/m2 suffering from chronic calculous cholecystitis. The comparative analysis of effects of anti-platelet agents of various pharmacologic groups in postoperative period after laparoscopic cholecys-tectomy was performed in 45 elderly and senile patients with excess body mass and obesity. The increase of platelets aggregation and hypercoagulation were established in patients in pre- and postoperative period. This proved the need for the pharmacological prophylaxis of thrombotic complications in surgical treatment of cholelithiasis.


EPIDEMIOLOGIC PECULIARITIES OF PYO-INFAMMATORY DISEASES IN PATIENTS WITH LUNG CANCER

In studies of epidemiology and peculiarities of the development of postoperative pyo-inflammatory diseases in patients operated for lung cancer it was established that these were pneumonia and pyothorax that developed more frequently. The incidence of postoperative complications does was not characterized by seasonal variations. Development of suppuration of postoperative wound is directly proportional to the number of postoperative pneumoniae and pyothorax.


ULTRASOUND CHARACTERISTIC OF VENOUS BLOOD FLOW OF LOWER EXTREMITIES IN VARICOSE DISEASE

485 patients were examined by ultrasound duplex scanning with color doppler carting of blood flow: 350 patients with various forms of varicose disease and 135 patients without any signs of venous pathology. Ultrasound characteristics of venous blood flow in varicose disease were obtained. The conclusions were drawn about the value of duplex scanning as a complex method of evaluation of lower extremities venous system enabling determination of anatomic peculiarities and disorders of phlebohemodynamics with topical characteristics. The method helps to choose rational treatment tactics based on maximal and sufficient correction of pathogenesis mechanisms of development of varicose disease and individual approach to each patient.


STATE OF HYPOPHYSIS-OVARIAN SYSTEM IN WOMEN WITH ENDOMETRIUM CANCER DURING POST MENOPAUSE

The hormonal status of hypophysis-ovary system, clinical peculiarities in 40 postmenopausal women with endometrial cancer were studied. Hormonal levels of the anterior hypophysis were determined. A considerable number of patients with endometrial cancer had disorders of menstrual cycle caused by anovulation and extragenital diseases. Significant differences in indices of anterior hypophysis and ovarian hormones were revealed. High relative and absolute predominance of estrogen serum content correlated with pathologic changes of endometrium.


CLINICAL EFFICACY OF FLUORESCENT CYSTOSCOPY IN INTRAOPERATIVE CONTROL OF COMPLETENESS OF TRANSURETHRAL RESECTION OF URINARY BLADDER TUMORS

Clinical evaluation of efficacy of fluorescent cystoscopy in intraoperative control of transurethral resection of urinary bladder tumors was performed. The results of the treatment of 15 patients aged from 43 to 76 years with superficial cancer of urinary bladder were analyzed. In all cases transurethral resection of urinary bladder was used as monotherapy without adjuvant immuno- and chemotherapy. Fluorescent cystoscopy sensitivity in diagnostics and control of completeness of transurethral resection of superficial tumors of urinary bladder was 97,5%. Observation was carried out up to 16 months. There were no relapses of tumors as detected by control fluorescent cystoscopy. Intraoperative fluorescent cystoscopy in transurethral resection of urinary bladder tumors makes it possible to improve efficacy and completeness of treatment.


ANALYSIS OF SKIN MICROBIOCENOSIS IN CHILDREN WITH ATOPIC DERMATITIS

Complications of atopic dermatitis by bacterial and fungous infection aggravates the persistence of atopic dermatitis in children causing resistance to traditional therapy. The results of cultural study of skin in 198 children with atopic dermatitis aged from 6 months to 15 years were analyzed. Colonization of skin by staphylococci both in insulated form and in associations with fungi and nonstaphylococcal flora prevailed. Bacteriologic and mycologic examination of skin in children with atopic dermatitis, complicated by secondary infection is necessary and provides differential approach to the treatment.


PECULIARITIES OF MICROCIRCULATION AND LIPID PEROXIDATION IN CHILDREN OF 8-10 YEARS OF AGE LIVING IN CONDITIONS OF TECHNOGENIC POLLUTION

Microcirculation and lipid peroxidation in children of 8-10 years of age living in various districts of Nizhnekamsk was evaluated with account of air pollution. The comparative analysis of quantitative - qualitative indices of bulbar conjunctiva in polluted district revealed significant disorders in microcircula-tion system as compared with indices in children living in relatively clean district. The urine levels of lipid peroxidation products in children of the polluted district was higher than in children in clean district by 31-32%. The results show toxic effects of air pollution on children living in the polluted districts of Nizhnekamsk.


ORGANIZATION OF PROPHYLACTIC EXAMINATION AND ITS MANAGEMENT IN MODERN CONDITIONS
