THE VARIANCES OF ECHOCARDIOGRAPHIC INDEXES IN CHILDREN WITH PRENATAL MATURATION DELAY AND CONGENITAL HEART DISEASE

Echocardiographic indexes of children with and without pre-natal maturation delay were evaluated in patients having several congenital heart diseases namely interatrial septum, secondary interventricular septum and open ductus arteriosus. Indexes reflecting hemodynamic changes of heart chamber sizes for each malformation were developed and adopted.


ISCHEMIC PRECONDITIONING AND PLATELET AGREGATION

The link between ischemic preconditioning and platelet agregation activity was studied in patients with stable angina pectoris. All patients had triple load bicycle ergometry. Ahead of third such bicycle ergometry patients passed remote ischemic preconditioning by imposing the balloon cuff on left forearm for 5 minutes. It was found that the bicycle ergometry test leads to decrease of platelet agregation in patients with ischemic heart disease. Ischemic preconditioning which occur after second bicycle ergometry test associated with more significant decrease of platelet agregation in patients taking aspirin. In patients not taking aspirin, more profound reduction of platelet agregation were found after remote ischemic preconditioning.


THE EXPERIENCE IN THE USE OF ENDOVASCULAR METHODS IN TREATMENT OF PATIENTS WITH ISCHEMIC HEART DISEASE

The direct and remote efficacies of two methods of coronary angioplasty were analysed. It was found that results of ballon dilatation and coronary stentation were depend on localisation, degree, prolongation of stenosis, morphology of atherosclerotic disorders of coronary arteries, presence of thrombus, calcinosis, number of arteries affected, type of myocardial hemocirculation, presence or absence of collateral circulation, condition of distal parts of coronary arteries. Coronary stentation improved prognosis in remote period, however do not reduced significantly relapse of ischemic conditions due to progress of coronary atherosclerosis.


PECULIARITIES OF NICOTINIC DEPENDENCY DEVELOPMENT IN PATIENTS WITH CHRONIC OBSTRUCTIVE PULMONARY DISEASE

140 patients (smokers and non-smokers) with chronic obstructive respiratory disease were under observation. Patients were taken selective M-cholinoblocker ipratropoium bromide (Atrovent H) for treatment of chronic obstructive respiratory disease and Nicorette for their nicotinic dependency. Clinical signs of chronic obstructive respiratory disease (cough, quantity and quality of sputum, breathlessness) were reduced and velocity indexes of external respiration were increased as results of treatment. After the treatment nicotinic dependency vanished in 17,1% of patients, reduced - in 68,4%, did not changed - in 7,9% and increased - in 2,6%.


VIDEOASSISTED TRANSAXILLARY SUBPECTORAL AUGMENTED BREAST RECONSTRUCTION

Four major types of operational incision approaches in augmented breast reconstruction have been described. The set of instruments developed by the company "Medpharmservice" for artificially created cavities was used with transcapillary incision approach. Transcapillary augmented mammoplasty is the optimal method for augmentation of a breast, while video-assistance allows full visual control over operation procedure. Moreover, this allows exclusion of cranial displacement of artificial implants and post-operation hemorrhages.


USAGE OF LEVITIRACETAM IN PREVENTIVE TREATMENT OF MIGRAINE WITHOUT AURA

The efficacy of an anticonvulsant, levitiracetam, to prevent of migraine without aura was studied. It was showed that the drug has multifactor actions, effective in small doses and safe during long treatment.


THE ROLE OF COMPUTER-TOMOGRAPHIC AND ULTRASONIC ANGIOGRAPHY IN STOMACH CANCER

112 patients with stomach cancer have been examined by computer-tomographic and ultrasonic angiography to evaluate the tumor invasion into the blood vessels. Four level of involvement of surgically important blood vessels into the tumor invasions were discriminated. Surgical data confirmed reliability of presurgical diagnosis in 110 case (98,2%).


THE ASSESSMENT OF AUDIONEUROLOGICAL, COGNITIVE AND HEMODYNAMIC DISORDERS IN CHILDREN OF EARLY AGE WITH ANTE- AND PERINATAL HYPOXIA

Multidiscipline assessment of the early aged children after ante- and prenatal hypoxia with and without hearing loss and disturbances of cerebral circulation was carried out. The performed neurological, audiological, psychological and neurophysiological examinations revealed the correlation between the rate of cognitive, motor disorders, hearing loss and the level of hypoxia and cerebral circulation deficit.


PEPTIC ULCER IN ELDERLY AND SENILE PATIENTS ASSOCIATED WITH ATHEROS CLEROTIC PATHOLOGY OF CARDIOVAS-CULAR SYSTEM

The peculiarities of stomach ulcer development were studied in 50 elderly and senile patients with the age of 55-77 y.o. who also had atherosclerotic pathology of cardiovascular system. Microcirculation condition, blood lipid spectrum and endothelial function were examined during exacerbation of ulcer. It was shown that in elderly and senile patients, ulcer has atypical clinical signs and often has complications. It was suggested that disturbances of microcirculation due to atherosclerosis of stomach blood vessels and endothelial dysfunction play important role in development of ulcer in elderly and senile patients.


FAKE DRUGS - A PROBLEM OF THE WORLD AND ITS SOLUTIONS

EXTRAPOLATION OF BIOSOCIOLOGICAL STRUCTURE OF A MAN ON PROFESSIONAL ACTIVITIES OF PHYSICIANS AND NURSES IN NARCOLOGY SERVICES

The opinions of psychiatrists, narcologists and nurses working in psychiatric and narcologic dispensers on biological, psychological, social and ethical components of their professional duties were analyzed. The levels of unification and mutual understanding between doctors and nurses were evaluated by anonymously testing 84 doctors and 87 nurses by the methods of unfinished sentences. Insufficient integration of different chains of narcologic health care system (doctors, nurses, administration, patients and their relatives) was found.


CATAMNESES STUDY OF OUT-PATIENT COHORT OF PSYCHIATRIC PATIENTS OF OVER 60

Catamneses of 527 patients registered in 1989 at the gerondopsychiatric doctor's consulting room at the Tver Regional Psychiatric clinic was analysed. It was found that during 13 years 2-10% of registered patients died annually, and by 2003 only 14% of all patients survived. Analysis the death rate in different groups of patients showed that higher mortality was in group with atrophic pathology of brain. A lower percentage of death rate was observed in the affective and delirious group.


THE ROLE OF HAIRDRESSING SALOONS AND COSMETOLOGY TECHNOLOGIES IN DISSEMINATION OF CONTACT (HEMO-CONTACT) INFECTIONS

OPTIMIZATION OF EARLY DIAGNOSIS OF ARTERIAL HYPERTENSION IN PREGNANCY

A new non-invasive method based on finger computer photoplatismography has been developed for evaluation of endothelial function. It was suggested that for diagnosis of endothelial dysfunction the dynamic of reflection index should be evaluated on the third minute of postischemic phase. It was found that in women with non-complicated pregnancy as well as in healthy non-pregnant women the reflection index is reduced during the test. In pregnant women with pregnancy-induced arterial hypertension this index is increased from the 14-th week of pregnancy, i.e. much earlier than clinical signs of the complication had developed. High sensitivity (86%) and specificity (75%) allow to recommend the method for early diagnosis of pregnancy-induced arterial hypertension.


FURTHER DEVELOPMENT OF THE METHODS OF DERMATOSURGICAL TREATMENT OF PRETUMOR CERVICAL DISEASES

It was found that results of the electrosurgical treatment of cervix uteri preneoplastic diseases depend on the form of the active electrode. The best results were obtained with an active electrode in the forms of round metallic loop.
