RESULTS OF DISTANCE LITHOTRIPSY DEPENDING ON THE PELVIS SYSTEM STRUCTURE

E.N. Sitdykov, M.E. Sitdykova, E.R. Aitova
The results of the treatment of patients with urolithiasis by the distance piezoelectric lithotripsy method for 10 years are analyzed. It is established that the efficiency of this method depends not only on the sizes and physicochemical structure of concrements but mostly on their localization, the pelvis system structure type, anatomic and functional state of kidneys and upper urinary tract. It is enough to carry out one procedure to destroy concrements of extrarenal pelvis, whereas the desintegration of concrements of intrarenal pelvis requires repeated procedures.

EFFICIENCY OF DIMEPHOSPHONUM, CYTOCHROM ‘ AND MEXIDOL IN THE TREATMENT OF CARDIAC RHYTHM DISORDERS IN CHILDREN

L.A. Balykova, M.I. Almyasheva
The efficiency of dimephosphonum, cytochrom c and mexidol in the treatment of ventricular, supra-ventricular arrhythmias in children is shown. Dime-phosphonum (100 mg/kg i.v.), mexidol (10 mg/kg p.o.) and cytochrom c (0,5 mg/kg i.v.) were studied in comparison with cordaron, finoptin and also with "the standard neurometabolic therapy". It is shown that dimephosphonum was the most effective drug in all kinds of disturbances of cardiac rhythm and conductivity in children. In supraventricular arrithmias and sinus node dysfunction it surpasses traditional drugs. At ventricular arrhythmias dimephosphonum was less effective than cordaron. However, cordaron induced cardiac (20%) and extracardiac (20%) negative effects. The dimephosphonum treatment was safe and was accompanied by favourable influence on the basic heart functions.

PUMP FUNCTION OF THE HEART IN PATIENTS WITH SCOLIOSIS IN REST AND IN STRESS TEST

Ya.Kh. Ibragimov, R.R. Nigmatullina
The indices of the pump function of the heart in teenagers with scoliosis of the 3d - 4 th degree in rest and during stress test are studied. Disorders of the pump function of the heart take place in rest: the heart rate signficantly increases. During physical exercise in patients with scoliosis the increase of minute blood volume is less pronounced than in healthy persons and is realized by the increase of the heart rate and not by the stroke blood volume. It is less economical form of adaptation of the pump function of the heart to physical exercise. The functional reserves for increasing the minute blood volume, volumetric circulation rate in the response to physical exercise are decreased.

COMPENSATING ADAPT€TION MECHANISMS OF THE CARDIORESPIRATORY SYSTEM OF SPORTSMEN OF DIFFERENT AGE GROUPS IN ECG STRESS TEST

Yu. S. Vanyushin, F.G. Sitdikov, M.Yu. Vanyushin
The mechanism of external respiration and gas exchange in sportsmen ECG stress test are studied. It is established that adaptation of the cardiorespiratory system to the increasing load in groups of sportsmen of different age is going through various ways. In groups of teenagers and adults aged 36-60 it occurs through pulmonary ventilation, in sportsmen aged 22-36 it occurs as a result of the increase of the oxygen use ratio, and in groups of youths - by the activation of gasotransportation system.

EFFICIENCY OF THE IRS-19 AEROSOL VACCINE IN CHILDREN WITH RELAPSING DISEASES OF UPPER RESPIRATORY TRACT

G.A. Makovetskaya, V.V. Zhestkova, T.L. Kuznetsova
The use of the IRS-19 aerosol vaccine results in decreasing of the rate of the infectious diseases of otorhinolaryngologic organs in children suffering from adenoids, sinusitis, complications of acute respiratory diseases.

METHODICAL APPROACH TO THE CLINICOPHARMACOTHERAPEUTIC DIFFERENTIATION OF SOMATOFORM DISORDERS

A.Yu. Zimakov, A.M. Karpov
The term "somatoform disorders" in general is similar to the concept of hypochondria. Seven components (sensory, cognitive, vegetative, affective, behavioral, temporal, therapeutic) making it possible to peform the clinical classification are shown.

NEW METHOD OF EPIDURAL ANESTHESIA IN CLOSED BLUNT CHEST INJURY

R.R.Safin, O.G. Anisimov, A.A. Nazipov
The thoracic epidural anesthesia is the most suitable method for the treatment of the closed blunt chest injurã but there is probability of the spinal cord injuiry. The original method combining the efficiency of classic thoracic epidural anesthesia with safety and simplicity of lumbar epidural anesthesia is suggested. This method is based on the postulates of the molecular hydrokinetic theory. Two catheters ends are disposed in lumbar epidural space in distance about two inches between them. Through one catheters end the anesthetic solution and through other's end the 0,9% saline are injected synchronously in equal volumes. Hydroplunger phenomenon provides the upward anesthetic solution spreading.

HUMORAL AND CELLULAR INDICES OF IMMUNITY IN NEWBORNS WITH EARLY REALIZATION OF THE ALLERGICALLY CHANGED REACTIVITY

A.G. Shamova, E.V. Agafonova, A.N. Stepanova, T.G. Malanicheva
The complex immunologic examination in 39 children aged 2 to 28 days with early realization of the allergically changed reactivity in boys - 61,5%, in girls - 38,5% is performed. The peculiarities of immunologic reactivity in children with early realization of the allergically changed reactivity (the increase of the immune regulatory index due to the deficiency of CD 8+ lymphocytes, the increased expression of activation markers, the increase of Ig E) show the prevalence of the atopic response type. Uniformity of changes of the immune status indices in early and late neonatal period testifies the intrauterine and/or early change of immuno-competent cell phenotype under the influence of perinatal development factors of a child.

TRANSBONE OSTEOSYNTHESIS BY THE ILIZAROV APPARATUS IN CALCANEAL FRACTURES

S.V. Krivoshapko
The new arrangement of the Ilizarov apparatus for treating calcaneal fractures is suggested. The scheme of laying wiring through calcaneum and its anatomotopographical grounds are shown. The nearest and remote results of treating 23 patients with calcaneal fractures are studied.

PECULIARITIES OF HEMODYNAMICS IN GESTOSIS

D.V. Sadchikov, D.V. Elyutin
As many as 239 pregnant women suffering from gestosis were examined and they were allowed to give birth by the caesarean operation. Nine types of hemodynamics were selected. All the patients were divided into two groups, depending on the level of gestosis. In each group two subgroups were picked out according to the type of therapy (the 1st one by the old way, the 2d one by new methods). It is shown that in both groups the hypertensive-hypodynamic dissociation and isolated hypertension prevailed. It is stated that in both groups the calculation of the Cardiac Index and Systemic Vascular Resistance with the following definition of the hypodinamic type reflects the severity of the course of gestosis and the effeciency of the therapy directed to the correction of hemodinamics. The application of suggested systematization of hemodynamics disorders and the therapy methods promotes quicker correction of complications of hypertensive therapy in the form of postural reactions than in the case of the routine method.

PATHOMORPHOLOGICAL ASPECTS OF THE PLACENTAL DEFICIENCY IN CHLAMIDIAL AND MYCOPLASMAL INFECTION

V.P. Nefedov, L.I. Maltseva, T.P. Zefirova, L.A. Valiullina
The peculiarities of morphogenesis of placenta lesions and pathogenesis of placental deficiency in these types of infectious pathology are studied. It is stated that the subcompensated form of the relative chronic placental deficiency in chlamidiosis can be developed, morphological vascularization indices of terminal villi being higher than normal and in mycoplasmosis such placental deficiency is connected with the partial decrease of vascularization indices. In the presence of mixed clamidia and mycoplasma infection in pregnants the dissociated type of villi lesions is observed and the placental vascularization indices (the content and state of capillaries, syncytiocapillar membranes and syncytial kidneys) are impaired to a greater extent resulting in chronic placental defficiency.

AVERAGE DURATION OF PATIENTS' INHOSPITAL STAY AND THE INCREASE OF WORK EFFICIENCY OF THE EMERGENCY SERVICE HOSPITAL

E.V. Lushnikov
It is established that the average duration of patients' inhospital stay varies depending on the season. Taking into account seasonal variations of the average duration of inhospital stay makes it possible to optimize the management process of medico-prophylactic institutions, gives the possibility to use the bed fund more rationally and gives more appreciable economic effect. The organization structure of dermatological service to rural population in Tatarstan Republic is given.

ORGANIZATION OF SURGICAL SERVICE IN FRANCE

A.M. Zainutdinov
The characteristic of structural subdivisions of Haury Mondor hospital in France, the organization of its surgical service, activity indices of the hospital are given. The peculiarities of work of physicians-interns, teaching of students are described, the merits in organization of medical service to patients, the quality of diagnosis and treatment are noted.

A CASE OF PSEUDOTUBERCULOSIS WITH LETHAL OUTCOME

N.M. Khakimov, K.Sh. Nizamutdinova, G.M. Sidimiriva
A rare case of pseudotuberculosis with lethal outcome in the child aged two months and 20 days is described. Y. pseudotuberculosis infection of the child occurred likely in using milk, for the second time contaminated by the pseudotuberculosis pathogene. The inhibition reaction of indirect hemagglutination is more sensitive method of revealing Y. pseudo-tuberculosis than the bacteriologic method.

LEPTOSPIROSES AS OCCUPATIONAL DISEASE

A.A. Nafeev, A.V. Merkulov, V.F. Pashkov, L.I. Zhadaeva
Leptospirosis incidence in representatives of various professional population groups (workers of meat processing factory, cattle-breeding) with estimation of the etiologic structure of diseases is studied. The dependence of purity of positive reactions on the length of work is proved. The presence of the high titer of specific antibodies found in persons with the increased risk of leptospirosis infection shows wider spread of leptospirosis than it is oficially registered. The specific measures to decrease infectiousness and occupational disease prevalence are suggested.

CHANGE OF IXODIDAE POPULATION IN CONNECTION WITH ANTHROPOGENIC TRANSFORMATION OF LANDSCAPES ON THE BOUNDARY OF FOREST AND FOREST-STEPPE ZONES OF THE MIDDLE VOLGA REGION

S.V. Alemasova, V.A. Boiko, N.I. Boroznov, O.K. Gracheva, G.I. Zybin, E.S. Zheleznova, E.G. Zybina
Within the last 60 years in the region studied the joint inhabitance zone of I. ricinus and I. persulcatus displaced in the North-East direction and the D. reticulatus natural habitat significantly expanded to the North-West. These regional changes depend on the temperature rise of climate, the decrease of precipitation and change of species and age of afforestation. Decay of epidemiologic stress of natural foci of the tick borne encephalitis, Lyme disease, and strengthening the epizootologic valency of foci of pyroplasmosis, nuttaliosis as well as epidemiologic importance of natural foci of tularemia of meadow-field type would be expected in further expansion of the boundaries of I. ricinus, D. reticulatus natural habitat and contraction of the inhabitance zone of I. persulcatus on the territories cleared of the latter species.
