SURGICAL TREATMENT OF CONGENITAL HEART DISEASES IN ADULTS

The results of treatment of 701 adult patients with congenital heart diseases from 1987 to 1997 are analyzed. As many as 248 operations with general postoperative lethality 3,2% are made. It is concluded on the necessity of improving the diagnosis of congenital heart diseases in children, especially in rural regions in relation to the high rate of mopportune revealing heart diseases resulting in the increase of the number of inoperable patients with pronounced pulmonary hypertension. The presence of symptoms or pronounced blood escape out of the left atricle into the right one in the absence of pulmonary hypertension is considered the indication to the radical correction of secondary defect of interatrial septum in adults. The radical heart disease correction in conditions of moderate hypotermy without artificial blood circulation is recommended in small secondary defect of interatrial septum.


CORRECTION OF ANTITHROMBOGENIC ACTIVITY OF THE VASCULAR WALL AND INSULIN PROVISION IN PATIENTS WITH ISCHEMIC HEART DISEASE

The antithrombogenic activity of the vascular wall during probe, insulin provision during the glucosotolerant test are studied in 50 patients with ischemic heart disease and in 18 healthy persons. The change of antiagregational, anticoagulant. fibrinolytic activity of the endothelial cells of the vascular wall is revealed. The essential hyperinsulinemia duruing the whole glucosotolerant test at the low level of C-peptide showing the disorder of transforming proinsulin into insulin is noted. The use of ticlide is recommended in combined treatment of patients with ischemic heart disease.


BRONCHOOBSTRUCTIVE SYNDROME IN PATIENTS WITH OUTPATIENT PNEUMONIA

The respiratory function test and biochemical blood indices - mucinous 3EG5 antigene and eosinophil cation protein showing the bronchi reactivity state are studied in patients with outpatient pneumonia. It is established that the pneumonia course is characterized by delay of normalization of velocity indices in relation to volume finally resulting in transformation of disorders of the mixed type into obstructive. The significant increase of the level of the eosinophil cation protein is highly informative and sensitive indicator of the increase of bronchi reactivity. More pronounced and prolonged increase of the level of mucinous antigene reflecting mucous hypersecretion, aggravation of bronchi drenage function and strengthening bronchical obstruction are characteristic for lingering course of pneumonia.


ENDOSCOPIC DRAINAGE OF ACUTE PURULENT ABSCESSES OF LUNGS USING RONKOLEIKIN

The endoscopic drainage of acute purulent abscesses of lungs is performed in 57 patients, in 27 of them using ronkoleikin. The good clinical effect is obtained: the length of stay in hospital, period of temperature normalization, period of sputum disappearance are reduced. The treatment results are improved. The immediate immunomodulating effect of ronkoleikin after its injection into the abscess covity is noted.


REMOTE RESULTS OF RADIOTHERAPY OF ESOPHAGUS CANCER

The results of radiotherapy of esophagus cancer in 127 patients are analyzed. It is noted that the life duration of patients depends on the incidence and localization of tumor process. The five-year survival of patients with esophagus cancer localized in the cervical and supetrhoracal parts of esophagus was 0, in the middle thoracal part was 3,2%, in the lower thoracal part was 11%, in all localizations was 4,7%. The immediate favourable result of radiotherapy was noted in 77% of the patients with esophagus cancer.


ANTIBODY PRODUCTION TO SPECIMENS OF MICROFLORA OF LARGE INTESTINE IN PATIENTS WITH B AND C VIRUS HEPATITIS

The changes of antibody concentration to specimens of the normal intestine microflora are revealed in conditions of disbiotic shifts and activated immune response in patients with B and C virus hepatites. Dysbacteriosis is the possible basis of the antibody level changes specific to some specimens of intestine microflora in B and C virus hepatites.


ENDOTOXINEMIA AND INTENSITY OF HUMORAL IMMUNITY IN SURGICAL TREATMENT OF ACUTE DISEASES OF EXTRAHEPATIC BILIFEROUS TRACTS

The level of systemic ¥ndotoxinemia and intensity of antiendotoxine humoral immunity in surgical treatment of acute diseases of extrahepatic bili rã tracts are studied. Two groups of patients were examined. The patients with acute cholecystitis complicated by mechanical jaundice were in the first group, the patients with various forms of acute cholecystitis without disorders of bile outflow were in the second group. It is established that the high level of systemic endotoxinemia is found in patients with signs of mechanical jaundice in preoperative period, in this case the level of general blood bilirubine is directly correlated with the level of systemic endotoxinemia in both groups of patients. The decrease of humoral antiendotoxine immunity in preoperative period and its recovery in postoperative period are noted in patients with mechanical jaudice phenomena as the level of systemic endotoxinemia decreases and bile passage into intestine is recovered.


EFFECT OF DISTANCE PIEZOELECTRIC NEPHROLITHOTRIPSY ON RENAL HEMODYNAMICS IN PATIENTS WITH UROLITHIASIS

The renal hemodynamics is studied in 50 patients before and after treatment using the distance piezoelectric nephrolithotripsy method by ultrasound scanning system in conditions of colour doppler and energy mapping. The reliable d¥crease of the index values to the initial indices occurred in two days after lithotripsy in patients with extrarenal structure of the calicle and pelvis system and only in seven days in patients with intrarenal structure in conditions of retaining the urine passage. The necessity of doppler-monitoring for determination the terms of performing regular lithotripsy seance is stressed taking into account the variation of the terms of recovering renal hemodynamics.


POSTOPERATIVE WOUND INFECTIONS: REASONS AND CONSEQUENCES

The results of the treatment of 4222 patients after planned (48,7%) and urgent (51,3%) surgical interventions are studied. Pyoinflammatory complications of postoperative wounds were developed in 539 patients. The reasons and risk factors of developing complications in this group of patients are determined by expert and sociologic investigations. The consequences of complications are estimated by the nature and terms of the treatment.


VIBRATORY THERAPY OF POSTIMMOBILIZATIONAL CONTRACTURES OF THE KNEE JOINT

For the treatment of postimmobilizational contractures of the knee joint and the development of the optimal method of vibratory therapy, the comparative analysis of results of the treatment of 49 patients aged 15 to 67 was carried out. In the basic group (29 patients) the vibration therapy action was performed instead of classical mechanotherapy (the frequency 12 hertz within the first 7-10 days of the treatment, 15 hertz within the following days, the vibration amplitude 2 mm; the vibratory action duration 5-7 min). Good results were obtained in 62,1% of the patients, satisfactory results were in 34,5% of the patients, unsatisfactory results were in 3,4% of the patients, in the control group (20 patients) 45%, 30% and 25%, respectively.


ELIZAROV ’RANSOSSEOUS OSTEOSYNTHESIS IN THE TREATMENT OF PATIENTS WITH CALCANEUS FRACTURES

The urgency of the problem of the treatment of calcaneus fractures is well-grounded. The peculiarities of the Elizarov transosseous osteosynthesis in the treatment of patients with various types of calcaneus fractures are shown. The analysis of remote results of the treatment of 50 patients with these fractures is carried out.


QUALITY OF LIFE AND COMPLIANCE WITH TREATMENT IN PATIENTS WITH ESSENTIAL HYPERTENSION

The relation of the total index of the quality of life and the integral index of compliance with treatment in definite levels of systolic and diastolic blood pressure as a whole by the example of all examined patients with hypertension, as well as in groups of persons with various minifestation of arterial hypertension with lesion and without lesion of brain vessels, is estimated. The obtained results of calculating the integral index of compliance with treatment in various levels of blood pressure, the estimation of its variation causes, as well as the correlation of this parameter with other characteristics show the difinite discriminative ability and validity of the method developed.


PERSONAL PSYCHOLOGIC PECULIARITIES OF ABORTED WOMEN

It is established that the significant part of aborted women is suscept ble to pronounced negative psychic reactions (personal alarm, psychotism, neurotism) which have stable nature. Women with such personal psychologic characteristics present a group of high risk of abortion propagation, various disorders of reproductive function and reproductive behaviour. In women dispensaries and in family planning service institutions it is necessary to introduce the psychologic testing to reveal persons of this risk group and to perform the corresponding psychologic correction for prevention of undesirable pregnancy.


EFFECT OF ANT…NATAL DEVELOPMENT OF DENTAL CARIES IN CHILDREN FACTORS ON THE

As many as 2551 families were examined in the conditions of the large industrial city. It is established that unfavourable factors of a mother during pregnancy correlate with 56 risk factors in a child aged 0-3. However the effect of ant¥natal factors decreases with age. These factors retain the correlation only with 26 risk factors of a child aged 12-15 (P<0,05).


ETIOLOGIC ROLE OF ACINETOBACTER BACTERIA IN DISEASES OF EARLY AGE GROUP CHILDREN

Seeding of Acinetobacter haemolyticus bacteria in newborns and children aged to one from 1997 to 1999 is analyzed. The growth of their seeding within the last three years is shown. The etiologic role of A. Haemolyticus bacteria in inflammatory processes in children is confirmed by the repeated secretion of the pathogene from the pathologic material during the disease and simultaneous revealing of microorganisms in patients with omphalitis-rhinitis, omphalitis-conjunctivitis, omphalitis-rhinitis-conjunctivitis. Cleansing of maternity hospital results in ceasing of Acinetobacter haemolyticus secretion in children.


USE OF THE "MYUSTELA" CREAM AS A CONTACT MEDIUM FOR ULTRAPHONOPHORESIS

The high efficiency of lidasa ultraphonophoresis using the "Myustela" cream as a basis in the treatment of 185 patients of orthopedotraumatologic profile is shown. A wide application of this method is recommended.
