CURRENT ASPECTS OF COMBINED TREATMENT OF PATIENTS WITH RHEUMATOID ARTHRITISThe results of prolonged monitoring (over15 years) of variations of microcirculation and peripheric blood flow in patients with rheumatoid arthritis are presented. The pathogenetic therapy methods using physical factors combined with medications treatment are developed and introduced. The new treatment method - diuciphone in dimethylsulphoxide - electrophoresis, correcting the regulation of the disordered state of microcirculation and peripheric blood flow, stabilizing the inflammatory process and clinical picture, improving the oxidation - reduction processes, is developed. Helium - neon laser emission in new doses in rheumatiod arthritis improves systolic inflow, increases main and collateral blood flow, linear rate of blood flow. The variable magnetic field of low frequency stimulates compensation processes of disorders of microcirculation and peripheric blood flow. Electromagnetic waves of superhigh frequency regulate introvascular status. EFFECT OF TRIMETAZIDINE ON THE FORMATION OF OXIDATIVE STRESS IN BLOOD OF PATIENTS WITH POSTINFARCTION CARDIOSCLEROSIS IN CORONARY ARTERY BYPASS OPERATIONSThe efficiency of trimetazidine in preventing oxydative stress developing in myocardium and blood after coronary artery bypass operations is studied. It is established that trimetazidine reactivated components of erythrocyte antioxidative mechanisms and substantially decreased the rate of membrane peroxydation. The preoperative treatment with trimetazidine can be used to protect blood cells and cardiomyocytes from reperfusion disorders in coronary artery bypass operations. INDICES OF THE CYTOGRAM OF BRONCHOALVEOLAR LAVAGE FLUID IN PATIENTS WITH CHRONIC OBSTRUCTIVE BRONCHITISThe results of studing 106 patients with chronic obstructive bronchitis study are given. A total of 12 practically healthy persons were examined as a control group. The indices of pulmonary function tests made it possible to divide all patients into six groups - males and females with nonsevere, moderate and severe chronic obstructive bronchitis. The study indices of inflammatory syndrome as well as cytograms of bronchoalveolar lavage fluid were analyzed in these groups of patients. It is established that the indices of inflammatory syndrome in exacerbation phase did not differ in patients with various disease types. The amount of neutrophils and alveolar macrophages in the cytogram of bronchoalveolar lavage both in male and in female depends on the disease phase. The necessity of antibacterial treatment should be controled by the level of neutrophils in the cytogram of bronchoalveolar lavage fluid. ELECTROGASTROENTEROGRAPHY IN THE DIAGNOSIS OF GASTROENTERIC TRACT DISEASESThe comparative analysis of efficiency of electrogastroenterography and other clinical and paraclinical diagnostic methods is performed. Electrogastroenterography can be used for noninvasive monitoring of the functional state of motor and evacuation function of gastroenteric tract during treatment. It allows to define the necessity of other diagnostic methods. The found changes of bioelectric activity in different gastroenteric tract organs make it possible to enrich the diagnostic data base of computed gastroenterography for optimisation of the diagnostic process. PECULIARITIES OF GASTROENTEROSTOMY HEALING IN MAGNETOLASER ACTIONIt is established that magnetolaser effect on the gastroenterostomy zone provides fast normalization of histamine and heparin content in anastomosis tissue, decrease of intensity of inflammatory and necrotic processes and stimulation of regenerative processes, improvement of microcirculation in anastomosis tissue, and gastroenterostomy healing in such conditions followed the pattern seen in first intention over the short postoperative term without coarse cicatrix formation. EXPERIENCE OF USING DISTANCE LITHOTRIPSY IN THE TREATMENT OF UROLITHIASIS ON THE "TRIPNER XI DIREX Ltd" APPARATUSIt is shown that the use of urgent distance lithotripsy in kidney stone disease is a pathogenetic method in treating pain syndrome, urodynamics recovery, a component of combined treatment and prevention of acute calculous obstructive pyelonephritis. Urgent distance lithotripsy decreases the number of instrumental and operative interventions and in absence of contraindications it should be considered a choice method in renal colic. The use of ultrasound study during distance lithotripsy allows to choose the most sparing for kidney parenchyma parameters of calculi cleavage regime and to prevent complications. EFFECT OF IRON-CONTAINING MEDICATIONS ON THE DEVELOPMENT OF GESTATIONAL PIELONEPHRITISThe urgency of studying pielonephritis in pregnancy, which is the result of high incidence of this pathology, and of the new relations in forming painful process are revealed. The increase of using iron preparations by pregnants can be one of the reasons of gestational pyelonephritis and nonefficiency of antibacterial therapy. The high level of hemoglobin in pregnancy - is the warning not only for a woman but above all for a fetus. CLINICOPATHOGENETIC TYPES OF ENDOMETRIUM HYPERPLASTIC PROCESSES IN WOMEN LIVING IN DIFFERENT ECOLOGICAL CONDITIONSThe effect of unfavourable ecological factors on pathogenesis of hyperplastic processes of endometry is studied. The concept of basic pathogenetic types of endometry pathology depending on technogenic stress and life conditions is formulated. Division of hyperplastic processes on pathogenetic types makes it possible to differentiate approaches to therapy at the preclinical stage in women living in conditions of increased technogenic stress and to perform detoxication measures preventing disease. PSYCHOLOGIC RISK FACTORS OF DEVELOPMENT OF ARTERIAL HYPERTENSION IN TEENAGERSThe psychologic factors leading to the development of arterial hypertension in teenagers are studied. Three principal clasters of emotional response are revealed. Low social adaptation, desadaptive emotional response in frustration situation and low frustration tolerance of teenagers with arterial hypertension show the presence of emotional instability and external passive stress comparing with healthy teenagers. The program of cognitive and behavioral psychotherapy is made taking into account the analysis performed for teenagers with arterial hypertension. IODINE DEFICIENCY AND PSYCHONEUROLOGICAL SHARACTERISTICS OF PUPILS OF SPECIAL SCHOOL FOR CHILDREN WITH MENTAL RETARDATIONPhysical development, intellectual characteristics, neurological status and severity of iodine deficiency were examined in 100 pupils: in 50 (the 1 st group) from the school for children with mental retardation and in 50 (the 2d group) from secondary school, aged 8 to 11 years. Median of urinary iodine level showed the presence of mild-to-moderate iodine deficiency in the 1 st group of children. In their physical and mental development children from special school showed retardation in comparison with secondary school pupils. Children from the 1 st group had symptoms of vegetative disfunction and coordination abnormalities more often than in the 2d group. It can be supposed that in children with mental retardation there is a mild-to-moderate iodine deficiency which can be one of the reasons determining neurological abnormalities and retardation in their mental and physical development. REMISSION DISORDERS IN PATIENTS WITH ALCOHOLISMIn examining patients with long-term (more than three years) remission by the massive statistical sample method (data for five years) the boundary psychic disorders, which were divided into hypochondrical, absessive - phobic, paranoiac and asthenic variants, are revealed in 150 pÁtients. Thus remission is a part of alcohol system, remission changes of a person are the result of premorbid and situation factors. In prolonged remission the role of situation factors decreases, personal deviations prevail, which in patients with alcoholism have common "remission" peculiarities manifesting themselves in enhancing inhibitory character features. COMPARATIVE EFFICIENCY OF ANESTHESIA BY ANALGETICS IN TREATING ALGETIC COMPONENT OF ABSTINENT SYNDROME IN PATIENTS WITH HEROIN ABUSEThe effeciency of various analgetics (tramadol, ketorolak, metamizol) in combined treatment of patients with heroin abuse is studied. It is shown that anesthetic effect of ketorolak compares well with tramadol and in the first days it even exceeds tramadol reducung the time and decreasing algetic syndrome intensity. The results obtained show the necessity of including ketorolak into the combined therapy of abstinent syndrome in patients with heroin abuse. ON THE CONTROL OF LABOUR CONDITIONS OF PERSONS WORKING WITH PERSONAL COMPUTERS, VIDEODISPLAY TERMINALS AND óïòYING DEVICESLabour conditions of persons working with personal computers, videodisplay terminals and ÓÏÒying devices are studied, in this case the effect of a variety of physical production factors and attendant chemical noxious substances on users is revealed. Discrepancies with sanitary standards and regulations on electromagnetic fields, illumination, ventilation, equipment location, air production medium composition are found. According to sanitary service instructions the works on rational construction of ventilation and conditioning, air, equipment grounding, illumination reconstruction, working place locations, monitor changing are performed. It is emphasized that control over labour conditions of users of personal computers and persons working with copying devices should be one of principal lines in the work of specialists on occupational hygiene. STATE OF BONE TISSUE AND CALCIUM-PHOSPHORUS METABOLISM IN WORKERS OF PHOSPHORUS INDUSTRYThe mineral density of bone tissue and indices of calcium-phosphorus metabolism in 103 men working at white phosphorus factory are studied by ultrasound densitometry of calcaneus. Three variants of bone changes are determined: normal, decreased and increased mineral density of bone tissue. The work on such factory results in the decrease of mineral density of bone tissue, and in 4,6% of cases - in the development of osteoporosis. The increased mineral density of bone tissue is one of manifestations of chronic intoxications by phosphorus. In all workers of phorphorus production bone formation is oppressed when insignificant resoption increases. EPIDEMIOLOGIC, MOLECULAR AND GENETIC CHARACTERISTICS OF HEMORRHAGIC FEVER WITH RENAL SYNDROME IN SAMARA REGIONEpidemiologic peculiarities of natural foci of hemorrhagic fever with renal syndrome of various type taking into account molecular and genetic characteristics of Khanta virus on the territory of Samara region are studied. The use of the claster analysis made it possible to divide the territory of Samara region into four zones which differ in the level and dynamics of disease incidence. The results allow to perform effectively preventive and antiepidemic measures in each zone. DEVICE FOR PLUNGING SOFT IMPLANTS INTO TISSUEThe data on improvement of the plunging method of epidermatoplasty are presented. For this purpose a new technology of making skin seedlings, as wel as a special device - implantator making it possible to perform one-moment cutting granular tissue and plunging free autodermal striae, is suggested. The encouraging results in the treatment of 17 patients are obtained. © 1995-2008 Kazan State University |
