Na+-Li+-COUNTERTPANSPORT IN CHRONIC FORMS OF HYPERTENSIVE ANGIOENCEPHALOPATHY IN MALES AND FEMALES

The rate of Na+-Li+-countertransport through the erythrocyte membrane in 85 patients with various forms of chronic hypertensive angioencephalopathy and 85 normotensive subjects is studied. On the basis of clinical neurologic and magnetoresonant tomographic studies of brain the groups of patients with the diffue form of hypertensive angioencep-halopathy, focal form of hypertensive angio-encephalopathy and arterial hypertension with focal or diffue lesions of brain substance were chosen. For the first time in multifocal and diffue forms of chronic hypertensive angioencephalopathy the diffe-rence of structural and functional state of cellular membranes, determined by the rate of Na+-Li+-countertransport through the erythrocyte membrane is established. The revealed relation of clinical and magnetoresonant tomographic manifestations of lesions of brain and the state of ion-transport functions of cellular membranes can be used for the development of methods of prediction, prevention and treatment of vascular lesions of brain.


IMMUNITY DISORDERS AND INFECTION COMPLICATIONS IN ELDERLY PATIENTS WITH END STAGE OF CHRONIC RENAL FAILURE BEING ON PROGRAM HEMODIALYSIS

The peculiarities of clinical manifestations of secondary infection and immunity disorders in 38 young patients in comparison with 48 elderly patients are studied for the purpose of subsequent increase of the treatment efficiency of patients older than 55 benig on program hemodialysis, prolongation and improvement of their life quality. The features of infection in living and died patients are revealed. The changes of immune protection of the organism depending on age, treatment time as compared with concentration of immunoglobulin and parathyroid hormone in blood are studied. Aging processes, treatment time of program hemodialysis have a negative effect on the immune response level.


AUTOIMMUNE VARIANT OF CHRONIC RELAPSING URTICARIA AND METHODS OF ITS DIAGNOSIS

A group of patients with chronic relapsing urticaria was examined. The autologic serologic cutiriaction test was performed by the procedure described by Hide M. with coauthors, the content of autoantibodies to thyroglobulin and thyroid peroxidase were determined in serum of patients. The positive autologic serologic cutireaction test was revealed in some patients, the increased content of autoantibodies to thyroglobulin and thyroid peroxidase as well as the increased level of general Ig E were found, in this case the increase of the latter was more often met in patients with mild form of the disease. Consequently, the autologic serologic cutireaction test as well as investigation of serum on the content of autoantibodies to hormones and thyroid gland tissue are recommended to patients with chronic relapsing urticaria to reveal the autoimmune variant of disease.


SYSTEMIC ENDOTOXINEMIA AND STRESS OF ANTIENDOTOXIN HUMORAL IMMUNITY DURING ENTEROSORPTION IN COMBINED TREATMENT OF ACUTE CHOLECYSTITIS AND MECHANICAL JAUNDICE

The effect of enterosorption on the level of systemic endotoxinemia and stress of antiendotoxin humoral immunity in combined treatment of acute diseases of biliferous tracts is studied. Two groups of patients were examined. Patients with acute cholecystitis, complicated by mechanical jaundice entered the first group, patients with various destructive forms of acute cholecystitis without disorder of bile outflow into intestine entered the second group. It is established that inclusion of enterosorbent (enterodesis) into the pre - and postoperative treatment program in patients with acute cholecystitis and its complications provided the reduction of endotoxin concentration in systemic blood flow and the recovery of antiendotoxin humoral immunity.


INVAGINATIONAL ESOPHAGODUODENOANASTOMOSIS IN GASTRIE CARCINOMA

Esophagoduodenoanastomosis is suggested when esophagus and duodenum anastomosed by double-row interrupted suture are invaginated into clear space of the latter, by three row interrupted suture between peritoneum with holding the muscular wall of the diaphragm esophageal foramen diaphragm crura on the one hand and seromuscular layer of duodenum - on the other hand. In this case load from esophageal-intestinal sutures is transferred to the suture line between diaphragm and duodenum wall. Food passage on duodenum is reserved, peritonization of anastomosis line is performed. The operation is carried out in one anatomic area, gastrie its duration is decreased.


SURGICAL TREATMENT OF CHRONIC ANTERIOR INSTABILITY OF THE KNEE JOINT

The experience of treatment of 27 patients with chronic injures of the anterior cruciform ligament of the knee joint using the exterior extraarticular reconstruction method is presented. The indications for taking into account the instability form of the knee joint and constitutional peculiarities of patients are determined.


ULTRASOUND HEMODYNAMICS PICTURE OF THE LIVER IN PREGNANTS

Peculiarities of blood flow in liver vessels in 80 pregnants with hestosis of various severity and in 75 patients with uncomplicated pregnancy are studied. The combined dynamic ultrasound examination, ultrasound fetometry dopplerometric examination of fetouteroplacental and hepatic blood flows, echocardiotocography of the fetus, control of indices of blood coagulability in dynamics are carried out. The typical change of hemodynamics of hepatic and fetouteroplacental basins, the increase of general peripheric resistance of vessels were noted in hestosis. This is typical for the most unfavourable hypokinetic hemodynamics type, in the presence of which fetoplacental insufficiency, intrauterine fetation arrest and such heavy complication as multipleorgan insufficiency develop.


CLINICO-MORPHOLOGIC PARALLELS IN THE ESTIMATION OF THE SEAR ON UTERUS AFTER CESAREAN SECTION

The morphologic estimation of the state of the sear on uterus after cesarean section is given and the risk factors of its incompetence in repeated pregnancy and labor are revealed. It is shown that postoperative pyoseptic complications in early postoperative period and medical abortions within the first year after cesarean section have a significant negative effect on the formation of incompetent sear on uterus. Threat of abor-tion in not a sign of sear incompetence on uterus and requires careful differential diagnosis. The favourable predict or of the labor by natural maternal passages is the thicknes of low uterus segment in sear area in maturity of pregnancy from 5 to 7 mm.


VARIANTS OF DISTURBANCE OXYGENDEPENDENT METABOLISM OF PHAGOCYTIZING BLOOD CELLS IN FREQUENTLY SICK CHILDREN AND WAYS OF ITS CORRECTION

Functional activity and functional reserve of phagocytizing blood cells were studied in 240 frequently sick children. The were 60 healthy children in a group of comparison. The immunologic methods and the method of luminol-dependent spontaneous and stimuluted chemoluminescence of blood were used. The immunologic data indicated the uniformity of this group, however the luminol-dependent chemilu-minescence indices differed. In treatment of children of these groups the medical drugs with differenty influencieg the generation of active forms of oxygen in phagocytizing cells were used. It made it possible to restore the reserve of phagocytizing cells, to normalize immunologic parameters, promoting the improvement and decrease of sickness rate in children.


CLINICO-ROENTGENOLOGIC PECULIARITIES OF CHILDREN BEING BORN WITH LARGE BODY MASS

Macrosomia problem stands out as one of the most actual problems in current obstetrics, perinatalogy and pediatry. The investigators studying the macrosomia problem unanimously mention the high percent of labor injuries in these children. The roentgenologic method makes it possible to provide a high level of diagnosis of perinatal injuries of central nervous system in children being born with large body mass. A total of 215 children being born with mass 4000 g and more, aged one month up to 15 years being on stationary treatment in the children hospital ¿8 with various pathology of central nervous system are the subjects of the study. The roentgenologic symptoms of injuries of skull, craniovertebral region and cervical part of vertebral column are described, the degrees of intracranial hypertension are distingished.


CLINICAL AND EPIDEMIOLOGICAL PECULIARITIES OF EL-TOR OGAVA CHOLERA

The data on cholera outbreak in Kazan are presented. A was total 52 patients with cholera and 18 vibriocarriers were observed. The disease caused by the El-Tor Ogava vibrio. The infection by vibrio took place through dirty water in Azino reservoir in 76,9% of persons during swimming, by contact way in family foci - in 23,1%. The disease course was mostly mild (in 69,3%), moderately severe (in 23%) and severe (in 7,7%), children aged up to 14 were 54,4%, aged 15 to 19 - 16,2%. The diagnosis was confirmed by secretion of El-Tor Ogava cholera vibrio. The treatment was combined etiotropic taking into account sensitivity of the vibrio to antibiotics and chemical preparations. Pathogenetic oral rehydration was carried out in 43% of persons, mixed (parenteral-oral) rehydration - in 27%.


ROENTGENORADIONUCLEID STUDIES OF THE FUNCTIONAL STATE OF PARAHASAL SINUSES AND PERMEABILITY OF THEIR ANASTOMOSES

Paranasal sinuses of 60 patients are studi by contrast roentgenography method and in 12 patients - by contrast catheterizational functional polysinusography. The quantity of secretion and the nature of inflam-matory process in paranasal sinuses, permeability of their anastomoses are determined. The diagnostic possibilities of dynamic scintigraphy in the imaging, the estimation of the functional state of ciliated epithelium of paranasal sinuses were revealed.


COMPARATIVE ESTIMATION OF ROENTGENOCEPHALOMETRIC AND COMPUTER TOMOGRAPHIC EXAMINATION OF MAXILLODENTAL-FACIAL ANOMALIES AND DEFORMATIONS

The roentgenocephalometric and computer-tomographic examination of 12 patients (2 male and 10 female) was performed. The comparative characteristic and possibility of combining teleroent-genic and computer tomography for qualitative and quantitative pathology of maxillodental-facial system is studied. The obtained results made it possible to elÁborate the corresponding tactics and strategy of treatment of anomalies and deforma-tions of maxillodental system.


VISUAL GENERATED POTENTIALS ON REVERSION OF CHESS FIELDS IN CHILDREN WITH NATAL DETERMINED MYOPIA

The most adequate method of investigatingof visual analyser is the study of reversive visual generated potentials. It is found that in children with natal determined myopia the state of indices of visual generated potentials largely depends on cerebral hemodynamics changes and to a lesser degree - on myopic refraction. Medicamental and manual therapy aimed on recovery of cerebral hemodynamics promote the stable increase of the amplitude of components of visual generated potentials and the improvement of visual analyser functions and are the most effective treatment methods in children with natal determined myopia.


ESTIMATION OF ANTIEPIDEMIC RELIABILITY OF FUNCTIONING THE TREATMENT AND PROPHYLACTIC INSTITUTIONS OF NABEREZHNYE CHELNY

To improve prophylaxis of intrahospital infections the estimation criteria of antiepidemic reliability of functioning the treatment and prophylactic institutions were developed. These criteria include 10 indices and are estimated in seores (with maximum - 100).
